伤科消肿止痛膏联合塞来昔布胶囊治疗急性痛风性关节炎及其炎症指标的影响

摘　　要：目的探究院内制剂伤科消肿止痛膏联合塞来昔布胶囊治疗急性痛风性关节炎(GA)的临床效果及其对于相关炎症指标的影响。方法选取2023年1月至12月赣州市中医院急诊科收治的120例寒湿痹阻型急性GA患者作为研究对象,按照随机数字表法分为中药组、西药组及联合组,每组各40例。依据纳入、排除标准及剔除标准筛选后,实际有113例患者参与研究,其中中药组38例,西药组36例,联合组39例。中药组采用伤科消肿止痛膏外敷治疗,西药组采用塞来昔布胶囊治疗,联合组采用伤科消肿止痛膏联合塞来昔布胶囊治疗,疗程为1周。比较三组患者的视觉模拟评分法(VAS)评分、红细胞沉降率(ESR)、血尿酸(BUA)、炎症因子[白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、C反应蛋白(CRP)]水平变化及中医证候积分、疗效。结果治疗后,三组患者的VAS评分、ESR、BUA、IL-6、TNF-α、CRP及中医证候积分低于本组治疗前,联合组各项指标低于中药组、西药组,差异有统计学意义(P<0.05)。联合组的总有效率高于中药组、西药组,差异有统计学意义(P<0.05)。结论伤科消肿止痛膏联合塞来昔布胶囊治疗急性GA疗效显著,可有效改善ESR及BUA水平,降低炎症因子水平,减轻患者疼痛感及中医症状。Objective To investigate the clinical effect of Shangke Xiaozhong Zhitong Ointment combined with Celecoxib Capsules in the treatment of acute gouty arthritis(GA)and its influence on related inflammatory indexes.Methods A total of 120 patients with cold-dampness impediment acute GA admitted to the Department of Emergency of Ganzhou Hospital of TCM from January to December 2023 were selected as the research objects,and they were divided into Chinese medicine group,western medicine group and combination group according to random number table method,with 40 cases in each group.After screening according to inclusion,exclusion and exclusion criteria,113 patients actually participated in the study,including 38 patients in the Chinese medicine group,36 patients in the western medicine group,and 39 patients in the combination group.The Chinese medicine group was treated with external application of Shangke Xiaozhong Zhitong Ointment,the western medicine group was treated with Celecoxib Capsules,and the combination group was treated with Shangke Xiaozhong Zhitong Ointment combined with Celecoxib Capsules for 1 week.Visual analogue scale(VAS)score,erythrocyte sedimentation rate(ESR),blood uric acid(BUA),inflammatory factors(interleukin-6[IL-6],tumor necrosis factor-α[TNF-α],C-reactive protein[CRP])levels,traditional Chinese medicine syndrome score and therapeutic effect were compared among the three groups.Results After treatment,VAS score,ESR,BUA,IL-6,TNF-α,CRP and traditional Chinese medicine syndrome score of the three groups were lower than those before treatment,and all indexes of the combination group were lower than those of the Chinese medicine group and the western medicine group,with statistical significances(P<0.05).The total effective rate of combination group was higher than those of Chinese medicine group and western medicine group,and the differences were statistically significant(P<0.05).Conclusion The combination of Celecoxib Capsules and Shangke Xiaozhong Zhitong Ointment in the treatment of acute GA is
