臭氧水穴位注射联合腔内冲洗对膝骨性关节炎患者疼痛及相关因子的影响

摘　　要：目的探讨臭氧水穴位注射联合腔内冲洗对膝骨性关节炎(KOA)患者疼痛及相关因子的影响。方法选取2023年6-12月河南中医药大学第三附属医院收治的64例老年KOA患者为研究对象,采用随机数字表法分为观察组和对照组,每组32例。对照组采用臭氧水穴位注射,观察组在对照组基础上联合臭氧水腔内冲洗,每周治疗1次,连续治疗4周。比较两组治疗前后视觉模拟量表(VAS)评分、Lysholm膝关节功能评分、临床疗效,以及关节液前列腺素E2(PGE2)、白细胞介素(IL)-1β、IL-6水平。结果两组治疗后VAS评分、Lysholm膝关节功能评分、PGE2、IL-1β及IL-6水平较治疗前均有明显改善(P<0.05),且观察组较对照组改善更明显(P<0.05)。多重线性回归分析显示,治疗前VAS评分与PGE2水平有关(P<0.05),与年龄、IL-6、IL-1β水平无关(P<0.05)。观察组总优良率明显高于对照组(P<0.05),两组患者在治疗过程中均未出现晕针、注射部位皮肤感染、症状加重、关节不适等不良反应。结论臭氧水穴位注射联合腔内冲洗治疗KOA的临床疗效较好,可能是通过调控致痛因子和炎症因子水平改善疼痛症状。Objective To explore the effect of ozone water acupoint injection combined intra-articular irrigation on pain and related factors in the patients with knee osteoarthritis(KOA).Methods Sixty-four elderly patients with KOA in the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from June to December 2023 were selected as the study subjects and divided into the observation group and control group by adopting the random number table method,32 cases in each group.The control group adopted the ozone water acupoint injection,and the observation group was combined with ozone water intra-articular irrigation on the basis of the control group.The both groups were treated once a week for 4 consecutive weeks.The Visual Analogue Scale(VAS)score,Lysholm knee function score,clinical effect and the levels of joint fluid PGE2,IL-1βand IL-6 before and after treatment were compared between the two groups.Results The VAS score,Lysholm knee function score and the levels of PGE2,IL-1βand IL-6 levels after treatment in the two groups were significantly improved compared with before treatment(P<0.05),moreover the improvement in the observation group was more significant than that in the control group(P<0.05).The multiple linear regression analysis showed that the VAS score before treatment was related with the PGE2 level(P<0.05)and had no relation with the age and level of IL-6 and IL-1β(P<0.05).The total excellence and good rate in the observation group was significantly higher than that in the control group(P<0.05).The no adverse reactions such as fainting during acupuncture treatment,skin infection at injection site,symptom increase and joint discomfort appeared during the treatment process.Conclusion The ozone water acupoint injection combined intra-articular irrigation in treating KOA has better effect,which may improve the pain symptom by regulating the induced pain factors and inflammatory factors levels.
