股骨近端防旋髓内钉内固定对老年股骨粗隆间骨折患者的疗效分析

摘　　要：目的:探讨股骨近端防旋髓内钉(PFNA)内固定在老年股骨粗隆间骨折(ITF)患者中的应用效果。方法:选择2021年6月-2023年6月沭阳县中医院收治的80例老年ITF患者为研究对象,按随机数字表法将患者分为试验组和对照组,各40例。试验组采用PFNA内固定治疗,对照组采用动力髋螺钉(DHS)治疗,两组均持续随访6个月。对比两组术后恢复情况、骨折复位质量、并发症发生情况。结果:与对照组相比,试验组术后引流量较少,首次下地负重时间、住院时间和骨折愈合时间均较短,差异均有统计学意义(P<0.05)。治疗后,试验组骨折复位质量评分高于对照组,差异有统计学意义(P<0.05)。两组并发症发生率比较,差异无统计学意义(P>0.05)。结论:PFNA内固定应用于老年ITF患者,可加快患者术后恢复速度,提升骨折复位质量,且安全性良好。Objective:To investigate the effect of proximal femoral anti-rotation intramedullary nail(PFNA)in elderly patients with intertrochanteric fracture(ITF).Methods:A total of 80 elderly ITF patients admitted to Shuyang County Hospital of Traditional Chinese Medicine from June 2021 to June 2023 were selected as research objects,and were divided into an experimental group and a control group according to random number table method,with 40 cases in each group.The experimental group was treated with PFNA internal fixation and the control group was treated with dynamic hip screw(DHS).Both groups were followed up for 6 months.The postoperative recovery,fracture reduction quality and complications were compared between the two groups.Results:Compared with the control group,the experimental group had lower postoperative drainage,the first weight-bearing time,hospital stay and fracture healing time were shorter,with statistical significances(P<0.05).After treatment,the score of fracture reduction quality in experimental group was higher than that in control group,the difference was statistically significant(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:The application of PFNA in elderly patients with ITF can accelerate the postoperative recovery speed,improve the quality of fracture reduction,and has good security.
