关节镜下锁定空心螺钉内固定治疗SchatzkeⅢ型胫骨平台骨折患者的效果观察

摘　　要：目的:观察关节镜辅助下锁定空心螺钉内固定治疗SchatzkeⅢ型胫骨平台骨折患者的临床效果。方法:选取2021年6月-2023年3月天津港口医院收治的62例SchatzkerⅢ型胫骨平台骨折患者为研究对象,依据随机数字表法将其分为锁定空心螺钉组和钢板内固定组,每组31例。两组均实施关节镜辅助下胫骨平台骨折复位内固定术,其中锁定空心螺钉组采用锁定空心螺钉内固定,钢板内固定组采用钢板内固定。比较两组膝关节解剖及功能恢复指标、围手术期指标和不良反应发生情况。结果:术前和术后1周,两组骨折分离距离比较,差异无统计学意义(P>0.05)。术前和术后1年,两组膝关节解剖及功能恢复指标比较,差异均无统计学意义(P>0.05)。与钢板内固定组比较,锁定空心螺钉组的术中出血量、住院费用均较少,手术时间、切口总长度均较短,术后3 d的VAS评分较低,差异均有统计学意义(P<0.05);两组住院时间和术后1周的VAS评分相比,差异均无统计学意义(P>0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:关节镜辅助下胫骨平台骨折复位内固定术中,与钢板内固定比较,采用锁定空心螺钉内固定在缩短患者手术时间及切口长度,减少治疗费用及术中出血量,术后短期减轻疼痛程度等方面更具有优势,但二者远期效果和不良反应发生率并无差异。Objective:To observe the clinical effect of arthroscopy-assisted locking hollow screw internal fixation in the treatment of Schatzke Ⅲ type tibial plateau fracture paten.Methods:A total of 62 patients with Schatzker type Ⅲ tibial plateau fracture admitted to Tianjin Port Hospital from June 2021 to March 2023 were selected as the study objects,and were divided into a locking hollow screw group and a plate internal fixation group according to random number table method,with 31 cases in each group.Arthroscopically assisted reduction and internal fixation of tibial plateau fracture were performed in both groups,the locking hollow screw group was used for locking hollow screw internal fixation,and the plate group was used plate for internal fixation.The anatomic and functional recovery indexes,perioperative indexes and the incidence of adverse reactions were compared between the two groups.Results:Before surgery and 1 week after surgery,there was no significant difference in fracture separation distance between the two groups(P>0.05).Before surgery and 1 year after surgery,there were no significant differences in knee functional recovery indexes between the two groups(P>0.05).Compared with the plate internal fixation group,the locked hollow screw group had less intraoperative blood loss,less hospitalization cost,shorter operation time and total incision length,and lower VAS score at 3 days after surgery,with statistical significances(P<0.05).There was no significant difference in length of hospital stay and VAS score 1 week after surgery between the two groups(P>0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion:In arthroscopy-assisted reduction and internal fixation of tibial plateau fracture,compared with plate internal fixation,locking hollow screw internal fixation has advantages in shortening the operation and hospital stay of patients and the length of incision,reducing the cost of treatment and the amount of intraoperative blood loss,and
