摘　　要：目的:探讨自体富血小板凝胶在慢性伤口创面愈合中的应用效果。方法:选取治疗符合入组的慢性伤口病人57例,按照随机数字表法分组,观察组29例,对照组28例,2组病人均按TIME原则处理伤口,在准备好伤口床后,对照组采用常规换药法治疗,观察组采用自体富血小板凝胶治疗,观察2组病人治疗14 d后伤口分泌物的细菌阳性率及治疗2个月后创面伤口的治疗效果(Ⅰ级伤口愈合率和总有效率)。同时比较2组伤口的愈合时间及治疗费用。结果:2组病人处置前伤口细菌阳性率差异无统计学意义(P>0.05),处置后14 d观察组伤口细菌阳性率低于对照组(P<0.05);治疗2个月观察组Ⅰ级伤口愈合率明显高于对照组(P<0.01),伤口治疗总有效率观察组明显高于对照组(P<0.01);观察组病人伤口愈合时间明显低于对照组(P<0.01);观察组病人治疗费用明显低于对照组(P<0.01)。结论:自体富血小板凝胶对慢性伤口细菌具有抑制作用,有助于提高慢性伤口创面愈合率,能缩短治疗时间,节省治疗费用,可推广使用。Objective:To investigate the effects of autologous platelet-rich gel in chronic wound healing.Methods:A total of 57 patients with chronic wounds were selected,and divided into the observation group(29 cases)and control group(28 cases)according to random number table method.The wounds of two groups were treated according to the principle of TIME.After the wound bed was prepared,the control group was treated with routine dressing change,and the observation group was treated with autologous platelet-rich gel.The bacterial positive rate of wound secretions after 14 days of treatment and efficacy of wound treatment after 2 months of treatment(gradeⅠwound healing rate and total effective rate)in two groups were observed.Meanwhile,the healing time and treatment cost between two groups were compared.Results:There was no statistical significance in the bacterial positive rate of wounds between two groups before treatment(P>0.05).The bacterial positive rate of wounds in the observation group was lower than that in the control group after 14 days of treatment(P<0.05).The wound healing rate in the observation group was significantly higher than that in the control group after two months of treatment(P<0.01).The total effective rate of wound therapy in the observation group was higher than that in the control group(P<0.01),the wound healing time in the observation group was significantly lower than that in the control group(P<0.01),and the treatment cost in the observation group was significantly lower than that in the control group(P<0.01).Conclusions:The autologous platelet-rich gel can inhibit chronic wound bacteria,help to improve the healing rate of chronic wounds,shorten the treatment time and save the cost of treatment,and it is worthy of use.
