动力加压钢板内固定与带锁髓内钉内固定治疗创伤性四肢骨折的效果观察

摘　　要：目的:分析动力加压钢板内固定与带锁髓内钉内固定治疗创伤性四肢骨折的临床效果。方法:抽取2015年1月—2022年8月广饶县人民医院收治的创伤性四肢骨折患者66例,随机分为对照组和观察组,每组33例。对照组采用动力加压钢板内固定治疗,观察组采用带锁髓内钉内固定治疗。对比两组手术相关指标、治疗效果、炎性因子水平及血液流变学指标。结果:观察组手术时间、住院时间、骨折愈合时间短于对照组,术中出血量少于对照组(P<0.05)。随访6个月,观察组治疗总优良率明显高于对照组(P=0.021)。术后,相较于对照组,观察组C反应蛋白、肿瘤坏死因子-α水平明显更低(P<0.05)。出院时,相较于对照组,观察组血浆黏度、全血高切黏度、全血低切黏度明显更低(P<0.05)。结论:创伤性四肢骨折患者运用带锁髓内钉内固定治疗的效果较好,可以改善手术相关指标,加快骨折愈合时间,缩短住院时间,减轻炎性反应和改善血液流变学指标。Objective:To analyze the clinical effect of internal fixation with power compression plate and internal fixation with locked intramedullary nails in the treatment of traumatic extremity fracture.Methods:Sixty-six patients with traumatic extremity fractures admitted to Guangrao County People's Hospital from January 2015 to August 2022 were extracted and randomly divided into the control group and the observation group,with thirty-three cases in each group.The control group was treated with power compression plate internal fixation,and the observation group was treated with locked intramedullary nail internal fixation.Surgery-related indexes,therapeutic effects,inflammatory factor levels and blood rheology indexes were compared between the two groups.Results:The operation time,hospitalization time,and fracture healing time of the observation group were shorter than those of the control group,and the intraoperative bleeding was less than that of the control group(P<0.05);at 6-month follow-up,the overall excellent rate of treatment of the observation group was significantly higher than that of the control group(P=0.021);after the operation,the levels of C-reactive protein and tumor necrosis factor-αof the observation group were significantly lower compared with those of the control group(P<0.05);at the time of discharge,compared with the control group,plasma viscosity,whole blood high cut viscosity and whole blood low cut viscosity water were significantly lower in the observation group(P<0.05).Conclusion:The use of internal fixation with locked intramedullary nails in traumatic extremity fracture patients has a better effect,which can improve the surgery-related indexes,accelerate the healing time of the fracture site,shorten the hospitalization time,reduce the inflammatory reaction and improve the blood rheology indexes.
