基于单支架植入和双支架植入行急诊经皮冠状动脉介入治疗急性心肌梗死冠状动脉分叉病变的临床效果比较

摘　　要：目的:探讨单支架和双支架植入行急诊经皮冠状动脉介入治疗急性心肌梗死(AMI)患者冠状动脉分叉病变的临床价值。方法:选取2021年8月—2023年8月广西桂东人民医院收治的50例AMI患者作为研究对象,依据单双号法随机分为研究组和对照组,各25例。对照组患者实施单支架植入行急诊经皮冠状动脉介入治疗(PCI),研究组患者实施双支架行急诊PCI。对比两组患者围术期指标、术后动脉造影、治疗效果以及生活质量。结果:与对照组相比,研究组PCI操作时间、术中X线透视长于对照组,造影剂用量多于对照组(P<0.05);干预后,研究组的分支动脉检查3级残余狭窄检出率较对照组更低(P<0.05);研究组的治疗总有效率高于对照组(P<0.05);干预后,研究组的各项生活质量评分较对照更高(P<0.05)。结论:相较于单支架植入,双支架植入行急诊PCI的效果更好,可改善围术期指标及术后动脉造影结果,提高患者生活质量。Objective:To investigate the clinical value of percutaneous coronary intervention with single stent and double stent implantation for acute myocardial infarction(AMI)patients.Methods:Fifty patients with AMI admitted to Guangxi Guidong People's Hospital from August 2021 to August 2023 were selected as the study objects and randomly divided into study group and control group according to odd-even number method,with twenty-five cases in each group.The control group underwent emergency percutaneous coronary intervention(PCI)with single stent implantation,while the study group underwent emergency PCI with double stents.Perioperative indexes,postoperative arteriography,therapeutic effect and quality of life were compared between the two groups.Results:Compared with the control group,the PCI operation time and intraoperative X-ray fluoroscopy in the study group were longer than those in the control group(P<0.05);after intervention,the detection rate of grade 3 residual stenosis in the main and branch arteries of the study group was lower than that of the control group(P<0.05);the total effective rate of the study group was higher than that of the control group(P<0.05);after intervention,the scores of quality of life in the study group were higher than those in the control group(P<0.05).Conclusion:Compared with single stent implantation,double stent implantation has better effect in emergency PCI,which can improve perioperative indexes and postoperative arteriography results,and improve patients'quality of life.
