摘  要:目的:调查早产儿0~24月龄体格生长状况,为指导本地区基层医疗机构做好早产儿保健提供依据。方法:选择2020年1月—2021年12月出生并在宿迁市宿豫区建卡管理的早产儿为研究对象,采用标准差离差法按年龄别体质量(WAZ)、年龄别体长(LAZ)、年龄别头围(HCZ)计算Z积分,分析评估追赶生长和营养发育特点。结果:WAZ、LAZ在正常出生体质量(NBW)组和低出生体质量(LBW)组呈现随月龄增大先快后慢增长趋势,6—8月出现峰值;极低出生体质量(VLBW)组随月龄增加持续上升,WAZ增长速度快于LAZ。HCZ各组呈现交替增长现象。WAZ、LAZ、HCZ等3个指标,8月龄前NBW组始终优于LBW组,LBW组始终优于VLBW组,统计学有差异(P<0.05)。24月龄追赶生长平均完成率不足60%。结论:出生体质量较出生体长更影响追赶生长,前6个月是追赶生长关键期,应定期干预提升追赶速度,合理保健减少成年慢性病发生。24月龄追赶完成率不高,应长期随访管理。Objective:To investigate the physical growth status of preterm infants from 0 to 24 months of age,and to provide a basis for guiding the primary healthcare organizations in this region to improve the healthcare of preterm infants.Methods:Preterm infants born from January 2020 to December 2021 and managed by card in Suyu District,Suqian City,were selected as the study subjects,and Z-scores were calculated by age-specific body mass(WAZ),age-specific body length(LAZ),and age-specific head circumference(HCZ)by the standard deviation separation method,and analyzed to assess the characteristics of catch-up growth and nutritional development.Results:WAZ and LAZ showed a trend of rapid and then slow growth with increasing months of age in the normal birth weight(NBW)and low birth weight(LBW)groups,with a peak in June-August;the very low birth mass(VLBW)group continued to rise with increasing age in months,and WAZ increased faster than LAZ;HCZ groups showed alternating growth phenomenon;WAZ,LAZ,HCZ and other three indicators,the NBW group was always better than the LBW group before 8 months of age,and the LBW group was always better than the VLBW group,and the difference in statistics was statistically significant(P<0.05);the average completion rate of catch-up growth at 24 months of age was less than 60%.Conclusion:Birth body mass affects catch-up growth more than birth body length;the first 6 months is the critical period for catch-up growth;regular interventions should be made to improve catch-up speed;reasonable health care should be taken to reduce the occurrence of chronic diseases in adults;the completion rate of catch-up at the age of 24 months was not high and should be managed by long-term follow-up.
