摘 要:目的基于MRI探讨孕中晚期胎儿肱骨长度和肱骨干比率与孕龄的相关性。方法横断面研究。纳入2018年5月—2023年7月青岛大学附属泰安市中心医院81例孕中晚期胎儿MRI检查的影像资料。孕妇年龄19~46(32.1±5.3)岁, 胎儿孕26~37+6(32.9±3.0)周。使用Siemens 1.5 T和3.0 T MR半傅立叶单次激发快速自旋回波或真稳态进动快速成像序列获取胎儿肱骨最大长轴位图像, 并测量肱骨长度(包括肱骨干长度、肱骨总长度), 计算肱骨干比率(肱骨干长度/肱骨总长度)。应用直线及曲线回归分析肱骨长度与孕龄的相关性, 并获得相应的最适回归方程, 采用Pearson相关分析法分析肱骨干比率与孕龄的相关性。结果 81例胎儿肱骨总长度为60.16~89.69(76.03±6.16)mm, 肱骨干长度为42.70~62.87(53.99±4.60)mm, 肱骨干比率为0.65~0.76(0.71±0.02)。肱骨长度与孕龄呈"S"曲线函数关系:肱骨总长度与孕龄的最适回归方程为Y=exp(5.11-25.51/孕周), R^(2)=0.805;肱骨干长度与孕龄的最适回归方程为Y^=exp(4.81-26.92/孕周), R^(2)=0.815。肱骨干比率与孕龄无相关性(r=0.113, P=0.238)。结论在孕中晚期, 胎儿肱骨长度与孕龄呈"S"曲线函数关系, 肱骨干比率保持相对恒定。Objective This study aimed to investigate the correlation among fetal humeral length,humerus shaft ratio,and gestational age(GA)in the second and third trimesters of pregnancy based on magnetic resonance imaging(MRI).Methods This cross-sectional study included the imaging data of 81 fetuses in the second and third trimesters of pregnancy undergoing MRI scans from May 2018 to July 2023 at the affiliated Tai'an City Central Hospital of Qingdao University.The maternal age was in the range of 19-46(32.1±5.3)years.The GA ranged 26-37+6(32.9±3.0)weeks.Siemens 1.5 T and 3.0 T MR Half-Fourier Acquisition Single-shot Turbo spin or True Fast Imaging with Steady-state Precession sequences were used to obtain the maximum long-axis images of the fetal humerus,and the humerus length(including that of the humerus shaft and the total humerus length)was measured.The humerus shaft ratio(humerus shaft length/the total humerus length)was calculated.The correlation between humerus length and GA was assessed through linear and curve regression analyses,and the corresponding optimal regression equations were obtained.Meanwhile,the correlation between the humerus shaft ratio and GA was analyzed via Pearson's correlation analysis.Results The total humerus length,humerus shaft length,and humerus shaft ratio ranged 60.16-89.69(76.03±6.16)mm,42.70-62.87(53.99±4.60)mm,and 0.65-0.76(0.71±0.02)mm in 81 fetuses,respectively.The humerus length showed an"S"curve functional relationship with GA,with the optimal regression equation for total humerus length and GA:Y=exp(5.11-25.51/GA),R^(2)=0.805.The optimal regression equation for the humerus shaft length and GA:Y^=exp(4.81-26.92/GA),R^(2)=0.815.No correlation was observed between the humerus shaft ratio and GA(r=0.113,P=0.238).Conclusion In the second and third trimesters of pregnancy,the humerus length of fetuses exhibited an"S"curve functional relationship with GA,and the humerus shaft ratio remained relatively constant.
