摘　　要：目的评估ABO血型不相容(ABO-incompatible,ABOi)与ABO血型相同/相容肝移植在儿童患者中的临床结果。方法本研究通过回顾性分析2017年5月23日至2023年4月30日期间,在湖南省儿童医院接受肝移植的49例儿童患者的临床资料。根据供受者ABO血型兼容性,分为ABOi组(8例)与non-ABOi组(41例)。对比两组患者的术前评分、移植等待时间、等待名单紧急程度、术后并发症以及生存率。结果ABOi组患者的术前PELD评分及等待名单紧急程度显著高于non-ABOi组,但两组在术后并发症和生存率方面差异无统计学意义(P>0.05)。ABOi组与non-ABOi组的术后生存率比较,差异也无统计学意义(P>0.05)。结论ABOi-LT在儿童患者中是一种安全有效的治疗方法。本研究支持ABOi LT作为扩展儿童肝移植供体库的有效策略,对于紧急程度高的患者尤其如此。Objective To evaluate the clinical outcomes of ABO-incompatible(ABOi)versus ABO-compatible/identical liver transplantation in pediatric patients.Methods This study conducted a retrospective analysis of clinical data from 49 pediatric patients who underwent liver transplantation at Hunan Children's Hospital between May 23,2017,and April 30,2023.Patients were divided into the ABOi group(8 cases)and the non-ABOi group(41 cases)based on ABO blood type compatibility between donors and recipients.The study compared pre-operative scores,
