基于病床工作效率法和床位产出效率的床位优化配置研究

摘　　要：目的:从病床工作效率以及产出效率对某医院2023年内外科情况做整体评估,为医院床位管理及相关医疗资源配置提供依据,使医院的医疗资源利用最大化。方法:提取样本医院2023年临床科室医疗质量统计报表中相关指标数据,运用病床工作效率指标计算各科室平均开放床位数95%的置信区间;从床位产出角度将床位有效RW值计算床位产出效率,兼顾工作效率与产出效率;为了方便统计,以病区为单位进行统计。结果:默认床位有效或者产出有效均可,所以被统计的88个临床科室中,27个科室床位设置合理,14个科室需要增加床位,46个科室需要适量减少床位。结论:新医改背景下,特别是随着DRG/DIP的支付体系逐步完善,床作为医院重要的投入资源之一,它的效率与产出发挥着重要的作用,医院应结合各专业临床特点动态调整床位,发展优势学科,加强双向转诊,床位资源共享,从而更有效地利用病床资源,创造良好的社会效益和经济效益。Objective:To evaluate the overall situation of internal and surgical departments in a hospital in 2023 based on bed working efficiency and output efficiency,providing a basis for hospital bed management and related medical resource allocation,thereby maximizing the utilization of medical resources.Methods:Relevant indicator data were extracted from the clinical department medical quality statistical reports of the sample hospital in 2023.The bed working efficiency index was used to calculate the 95%confidence interval of the average number of open beds in each department.From the perspective of bed output,the effective RW value of beds was calculated to determine bed output efficiency,considering both working efficiency and output efficiency.For ease of statistics,the analysis was conducted by ward.Results:Assuming either bed effectiveness or output effectiveness,among the 88 clinical departments analyzed,27 departments had a reasonable bed allocation,14 departments needed to increase beds,and 46 departments needed to moderately reduce beds.Conclusion:Under the new medical reform context,particularly with the gradual improvement of the DRG/DIP payment system,beds,as one of the hospital's important resources,play a crucial role in efficiency and output.Hospitals should dynamically adjust bed allocations based on the clinical characteristics of each specialty,develop advantageous disciplines,strengthen two-way referrals,and promote bed resource sharing,thereby more effectively utilizing bed resources and creating good social and economic benefits.
