集采药盐酸罗哌卡因用于剖宫产的最佳有效剂量研究

摘　　要：目的:研究集采药盐酸罗哌卡因用于剖宫产的最佳有效剂量。方法:选取2022年3月—2023年3月在山西省汾阳医院进行剖宫产的产妇60例,采用随机数字表法将其分为单纯组(单纯采用盐酸罗哌卡因)和复合组(采用盐酸罗哌卡因复合舒芬太尼)两组各30例。比较两组收缩压、心率、不良反应和新生儿1min Apgar评分,计算两组半数有效量(ED50)及其95%可信区间。结果:两组不同时间收缩压、心率及两组注药后5min、注药后10min、出室时心率与入室时比较,差异均无统计学意义(P>0.05)。两组注药后5min、注药后10min、出室时收缩压与入室时比较均降低,差异有统计学意义(P<0.05)。两组不良反应总发生率、新生儿1min Apgar评分比较,差异无统计学意义(P>0.05)。单纯组盐酸罗哌卡因的ED50为14.09mg[95%可信区间(13.69,14.49)],复合组盐酸罗哌卡因的ED50为9.70mg[95%可信区间(9.34,10.07)]。结论:单纯采用盐酸罗哌卡因和采用盐酸罗哌卡因复合舒芬太尼均可满足剖宫产麻醉,但复合舒芬太尼时可减少盐酸罗哌卡因使用剂量,且不增加不良反应。Objective To study the optimal effective dose of collector drugropivacaine hydrochloride for caesarean delivery.Methods Sixty women who underwent cesarean section in Fenyang Hospital of Shanxi Province from March 2022 to March 2023 were selected and divided into simple group(Ropivacaine Hydrochloride only)and compound group(Ropivacaine Hydrochloride combined with Sufentanil)with 30 cases in each group by random number table method.Systolic blood pressure,heart rate,adverse reactions and 1 min Apgar score were compared between the two groups,and the 50%effective dose(ED50)and 95%confidence interval were calculated between the two groups.Results There were no significant differences in systolic blood pressure and heart rate between the two groups at different times,and heart rate between 5 min after injection,10 min after injection,at the time of leaving room and at the time of entring the room(P>0.05).The systolic blood pressure at 5 min after injection,10 min after injection,and at the time of leaving the room was lower than that at the time of entering the room,and the differences were statistically significant(P<0.05).There were no significant differences in the tatol incidence of adverse incidence and 1-min Apgar score between the two groups(P>0.05).The ED50 of ropivacaine hydrochloride was 14.09 mg[95%confidence interval(13.69,14.49)]in the simple group and 9.70 mg[95%confidence interval(9.34,10.07)]in the combined group.Conclusions Both Ropivacaine Hydrochloride and Ropivacaine Hydrochloride combined with Sufentanil can satisfy cesarean section anesthesia,but combined with Sufentanil can reduce the dose of Ropivacaine Hydrochloride,and do not increase adverse reactions.
