摘  要:目的探讨高频震荡通气联合氨溴索治疗新生儿胎粪吸入综合征(MAS)的效果和对患儿氧合功能和炎症反应的影响。方法选取2019年8月至2022年8月上饶市立医院新生儿科收治的76例MAS患儿作为研究对象,按红蓝球法分为观察组与对照组,每组38例。对照组仅给予氨溴索治疗,观察组采用高频震荡通气联合氨溴索治疗,比较两组临床疗效、氧合功能和炎症指标。结果观察组治疗总有效率高于对照组,但差异无统计学意义。治疗后,两组动脉血氧分压(PaO_(2))高于治疗前,动脉血二氧化碳分压(PaCO_(2))、平均气道压(MAP)、氧合指数(OI)均低于治疗前,且观察组PaO_(2)高于对照组,PaCO_(2)、MAP、OI均低于对照组,差异有统计学意义(P<0.05)。治疗后,两组C反应蛋白(CRP)、降钙素原(PCT)、白细胞介素-6(IL-6)水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。观察组住院时间、氧疗时间和肺部啰音消失时间均短于对照组,差异有统计学意义(P<0.05)。两组不良反应发生率比较差异无统计学意义。结论高频震荡通气联合氨溴索治疗新生儿MAS疗效确切,可快速改善患儿氧合功能,减轻机体炎症反应,安全性较高,值得临床推广应用。Objective To observe the effect of high-frequency shock ventilation combined with amiloride in neonatal meconium aspiration syndrome(MAS)and its effect on oxygenation function and inflammatory response.Methods 76 cases of children with MAS admitted to the Neonatal Unit of Shangrao Municipal Hospital from August 2019 to August 2022 were selected as the research subjects,and they were divided into the observation group and the control group according to the red and blue ball method,with 38 cases in each group.The control group was treated with ambroxol only,the observation group was treated with high-frequency shock ventilation combined with amiloride,the clinical efficacy,oxygenation function and inflammatory response were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the arterial partial pressure of oxygen(PaO_(2))level of the two groups was higher than that before treatment,and the arterial partial pressure of carbon dioxide(PaCO_(2)),mean airway pressure(MAP),oxygenation index(OI)of the two groups was were lower than those before treatment,and PaO_(2) in the observation group was higher than that in the control group,and the PaCO_(2),MAP and OI were lower than those in the control group,and the differences were statistically significant(P<0.05).After treatment,the levels of C-reactive protein(CRP),procalcitonin(PCT)and interleukin-6(IL-6)of the two groups were lower than before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).The hospitalization time,oxygen therapy time and moist rales disappearance time in the observation group were shorter than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups.Conclusion High-frequency shock ventilation combined w
