摘　　要：目的调查老年脑卒中患者口腔衰弱现状并分析影响因素。方法选择2023年3—12月芜湖市某三级甲等综合医院住院的226例老年脑卒中患者作为调查对象,采用一般资料问卷、口腔衰弱筛查量表-8、FRAIL量表、营养风险筛查2002、Barthel指数、老年口腔健康评价指数(GOHAI)进行数据收集,采用Logistic回归分析老年脑卒中患者口腔衰弱的影响因素。结果老年脑卒中患者口腔衰弱高风险138例(61.5%),文化程度、既往慢性病数量、NIHSS评分、Barthel指数、身体衰弱、营养状况、口腔健康评价指数是老年脑卒中患者口腔衰弱的影响因素(P<0.05)。结论老年脑卒中患者口腔衰弱发生率较高,文化程度低、NIHSS评分高、身体衰弱、营养不良、慢性病既往史超过2个,自理能力低下、口腔健康情况差的老年脑卒中患者发生口腔衰弱的风险高,临床医护人员应多关注老年脑卒中患者口腔衰弱问题,及时采取个体化的干预措施,为临床实践提供更为全面且精准的参考依据。Objective To investigate the current status of oral frailty among elderly stroke patients and analyze its influencing factors.Methods From March to December,in 2023,226 elderly stroke patients who were admitted to a tertiary grade A comprehensive hospital in Wuhu City,Anhui Province,were selected as the respondent.General information questionnaires,the Oral Frailty Screening Scale-8,FRAIL scale,Nutrition Risk Screening 2002,Barthel Index,and Geriatric Oral Health Assessment Index(GOHAI)were used for data collection.Logistic regression analysis was employed to investigate the influencing factors of oral frailty in elderly stroke patients.Results There were 138 cases of elderly stroke patients with high risk of oral frailty,and the prevalence was 61.5%.Level of education,the number of history of chronic diseases,NIHSS score,Barthel Index,physical frailty,nutritional status,and Geriatric Oral Health Assessment Index were identified as influential factors of the oral frailty among elderly stroke patients(P<0.05).Conclusions The occurrence rate of oral frailty is high among elderly stroke patients.Elderly stroke patients those with low educational levels,high NIHSS scores,physical frailty,malnutrition,more than 2 chronic diseases history,low self-care ability,and poor oral health are at a higher risk of developing oral frailty.Clinical healthcare providers should pay more attention to the issue of oral frailty in elderly stroke patients,and promptly implement personalized intervention measures,in order to provide a more comprehensive and precise reference basis for clinical practice.
