性激素、抗苗勒管激素联合超声对不孕症女性卵巢储备功能的评估价值

摘　　要：目的:探讨性激素、抗苗勒管激素(AMH)联合超声对不孕症女性卵巢储备功能的评估价值。方法:回顾性分析2020年5月—2022年6月江西中医药大学第二附属医院91例不孕症患者,根据获卵数将其分为卵巢正常反应组54例(获卵数>5个),卵巢低反应组37例(获卵数≤5个)。同期选择65例样本医院行体检的健康女性作为健康对照组。比较3组受试者基础性激素、平均卵巢体积(OV)、平均窦卵泡个数(AFC)及AMH。比较不同方法(基础性激素、AMH、超声以及三者联用)评估卵巢储备功能下降情况。结果:卵巢正常反应组和卵巢低反应组血清基础卵泡刺激素(b FSH)、b FSH/黄体生成素(LH)、雌二醇(E_2)高于健康对照组,且卵巢低反应组高于卵巢正常反应组,差异有统计学意义(F=195.426、186.438、228.218,P<0.05)。卵巢正常反应组和卵巢低反应组OV、AFC、AMH低于对照组,且卵巢低反应组低于卵巢正常反应组,差异有统计学意义(F=279.649、732.767、99.424,P<0.05)。三者联用的灵敏度(94.58%)、特异度(92.59%)及准确度(93.41%)均高于其他3种方法,差异有统计学意义(χ^(2)=8.706、9.103、17.690,P<0.05)。结论:不孕症女性的基础性激素高于正常水平,且其OV、AFC、AMH低于正常水平。性激素、AMH水平联合经阴道三维超声能够有效评估不孕症女性卵巢储备功能。Objective:To explore the value of sex hormones,anti-mullerian hormone(AMH)combined with ultrasound in the evaluation of ovarian reserve function in infertile women.Methods:A retrospective analysis was made on 91 infertile women in the hospital from May 2020 to June 2022.According to the number of oocytes retrieved,they were divided into 54 patients with normal ovarian response(>5 oocytes retrieved)and 37 patients with low ovarian response(≤5 oocytes retrieved).At the same time,65 healthy women who underwent physical examination in the hospital were selected as the healthy control group.The basic sex hormones,mean ovarian volume(OV),mean sinus follicle count(AFC)and anti AMH were compared among the three groups.Different methods(basic sex hormone,AMH,ultrasound and their combination)were compared to assess the decline of ovarian reserve function.Results:The levels of bFSH,bFSH/LH and E2 in normal ovarian response group and low ovarian response group were higher than those in healthy control group(F=195.426,186.438,228.218;P<0.05).OV,AFC and AMH in normal ovarian response group and low ovarian response group were lower than those in control group,and those in low ovarian response group were lower than those in normal ovarian response group.The difference was statistically significant(F=279.649,732.767,99.424;P<0.05).The sensitivity(94.58%),specificity(92.59%)and accuracy(93.41%)of the combination of the three methods were higher than those of the other three methods,and the difference was statistically significant(χ^(2)=8.706,9.103,17.690;P<0.05).Conclusion:The basic sex hormones of infertile women are higher than those of normal people.OV,AFC and AMH are lower than those of normal people.The levels of sex hormone,AMH and transvaginal three-dimensional ultrasound can effectively evaluate the ovarian reserve function of infertile women.
