摘　　要：肝衰竭(Liver Failure,LF)是临床急危重症之一,病死率高,严重危胁我国人民的健康,是目前亟待解决的医学难题。细胞焦亡是炎性小体介导的细胞程序性坏死,炎性小体激活半胱氨酸蛋白1(caspase-1)诱导肝细胞焦亡,释放大量促炎因子导致LF,其中炎症因子在LF发病中起重要作用。中医核心病机为“湿、毒、瘀”,细胞焦亡过程产生的炎性介质可视为中医“湿、毒、瘀”的微观体现。本文通过“湿毒瘀”病机理论探讨细胞焦亡在LF免疫炎症损伤中的作用,以期为中西医结合防治LF提供科学的理论依据和新思路。Liver failure(LF)is one of the clinical acute and critical diseases,with high fatality rate and serious threat to the health of Chinese people.It is an urgent medical problem to be solved at present.The core pathogenesis of traditional Chinese medicine is“dampness,toxin and blood stasis”.Pyroptosis is inflammasome-mediated programmed cell necrosis.Inflammasome activation of cysteine protein 1(caspase-1)induces hepatocyte pyroptosis and releases a large number of proinflammatory factors leading to LF,among which inflammatory factors play an important role in the pathogenesis of LF.The inflammatory media produced in the process of cell pyroptosis can be regarded as the microscopic embodiment of“dampness,toxin and blood stasis”in traditional Chinese medicine.Therefore,this paper discussed the role of pyroptosis in LF immune-inflammatory injury through the pathogenesis theory of“dampness,toxin and blood stasis”,which can provide scientific theoretical basis and new ideas for the prevention and treatment of LF with combined traditional Chinese and western medicine.
