基于气血理论探讨囊性变在激素性股骨头坏死塌陷和修复中的意义

摘　　要：激素性股骨头坏死(SIONFH)是目前骨科领域的疑难病,“坏死-修复-塌陷-骨关节炎”是SIONFH患者逐步演变的自然病理过程,其中修复、塌陷对预后的影响至关重要,团队前期研究发现囊性变具有修复能力,可形成新骨阻止塌陷的发生,可能是激素性股骨头坏死修复的核心反应区。目前,中医药早期治疗在临床中具有良好疗效,在SIONFH保髋的价值越来越受到重视。前期研究发现血瘀是贯穿激素性股骨头坏死发病过程的重要病机,《素问·评热病论篇》言:“邪之所凑,其气必虚”,激素性股骨头坏死亦与气虚有着密切的联系,本文结合气血理论,拟探讨囊性变在激素性股骨头坏死塌陷和修复中的重要意义,以期为中医药早期治疗激素性股骨头坏死提供参考。Steroid-induced osteonecrosis of the femoral head(SIONFH)is a difficult disease inorthopedics.“Necrosis-repair-collapse-osteoarthritis”is a natural pathological process that gradually evolves in patients with SIONFH.The effect of repair and collapse on prognosis is crucial.The previous study of the team has found that cystic degeneration has the ability to repair and can form new bone to prevent the occurrence of collapse,which may be the core reaction area for the repair of steroid-induced osteonecrosis of the femoral head.At present,the early treatment of traditional Chinese medicine has a good effect in clinical practice,and the value of hip preservation in SIONFH is getting more and more attention.Previous studies have found that blood stasis is an important pathogenesis throughout the pathogenesis of SIONFH.Huangdi Neijing said:“where the evil comes together,its Qi should be deficient”.Steroid-induced osteonecrosis of the femoral head is also closely related to qi deficiency.In this paper,combined with the theory of Qi and blood,it intends to explore the significance of cystic degeneration in the collapse and repair of SIONFH,in order to provide reference for the early treatment of SIONFH with traditional Chinese medicine.
