千金苇茎汤合透脓散加减对肺部感染患者通气功能、血气指标和微炎症状态的影响

摘　　要：目的观察千金苇茎汤合透脓散加减对肺部感染患者通气功能、血气指标和微炎症状态的影响。方法选取2020年1月—2023年1月医院收治的60例肺部感染患者为研究对象,依据随机数字表法分为对照组和观察组各30例,对照组给予西医常规治疗,观察组在对照组基础上给予千金苇茎汤合透脓散加减治疗,比较两组临床疗效、中医证候积分、临床肺部感染评分(CPIS)、肺功能[一秒钟用力呼气容积(FEV1)、每分钟最大通气量(MMV)、肺总量(TLC)]、血气指标[氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))、血氧饱和度(SaO_(2))]、微炎症状态[降钙素原(PCT)、C反应蛋白(CRP)、白细胞介素-6(IL-6)、白细胞计数(WBC)]及不良反应发生率。结果观察组治疗有效率高于对照组(93.33%vs 73.33%)(P<0.05)。治疗后两组中医证候积分、CPIS评分、PaCO_(2)和PCT、CRP、IL-6、WBC水平均降低,FEV_(1)、MMV、TLC、PaO_(2)、SaO_(2)均升高(P<0.05),且观察组中医证候积分、CPIS评分、PaCO_(2)和PCT、CRP、IL-6、WBC水平低于对照组,FEV_(1)、MMV、TLC、PaO_(2)、SaO_(2)高于对照组(P<0.05)。观察组不良反应发生率低于对照组(3.33%vs20.00%)(P<0.05)。结论千金苇茎汤合透脓散加减治疗肺部感染患者的疗效显著,可改善中医证候、缓解病情,提高肺功能,平衡血气指标,减轻炎症反应,且安全性较高。Objective To observe the effect of modified Qianjin Weijing Decoction(千金苇茎汤)and Tounong Powder(透脓散)on ventilation function,blood gas indexes and micro-inflammation status in patients with pulmonary infection.Methods A total of 60 patients with pulmonary infection admitted to the hospital from January 2020 to January 2023 were selected as the study subjects.They were divided into the control group and the observation group by random number table method with 30 cases in each group.The control group was treated with conventional western medicines,and the observation group was treated with modified Qianjin Weijing Decoction and Tounong Powder on this basis.Clinical effects,TCM syndrome scores,clinical pulmonary infection score(CPIS),pulmonary function[forced expiratory volume in 1 second(FEV_(1)),maximum minute ventilation(MMV)and total lung capacity(TLC)],blood gas indexes[partial pressure of oxygen(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2))and blood oxygen saturation(SaO_(2))],micro-inflammation status[procalcitonin(PCT),C-reactive protein(CRP),interleukin-6(IL-6)and white blood cell count(WBC)]and the incidence of adverse reactions were compared between the two groups.Results The treatment response rate in the observation group was higher than that in the control group(93.33%vs.73.33%)(P<0.05).After treatment,TCM syndrome scores,the levels of CPIS,PaCO_(2),PCT,CRP,IL-6 and WBC in both groups decreased.The levels of FEV_(1),MMV,TLC,PaO_(2)and SaO_(2)increased(P<0.05).TCM syndrome scores,the levels of CPIS,PaCO_(2),PCT,CRP,IL-6 and WBC in the observation group were lower than those in the control group.The levels of FEV_(1),MMV,TLC,PaO_(2)and SaO_(2)were higher than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(3.33%vs.20.00%)(P<0.05).Conclusion Modified Qianjin Weijing Decoction and Tounong Powder can achieve significant therapeutic effect on patients with pulmonary infection,which can improve TCM sy
