补阳还五汤对气虚血瘀型脑出血脑血流参数的影响

摘　　要：目的观察补阳还五汤对气虚血瘀型脑出血患者脑血流参数的影响。方法2020年1月—2022年6月,在湖南省脑科医院治疗的80例气虚血瘀型基底节-丘脑区脑出血患者随机分为试验组和对照组,每组各40例。试验组在常规药物治疗基础上于入院第2天加用补阳还五汤14 d。在入院24 h内、21 d采用磁共振灌注加权成像(PWI)观察两组血肿最大层面外缘靠皮层下1cm脑血流参数变化。结果治疗21 d后,试验组血肿最大层面外缘靠皮层下1cm局部脑血流量(57.05±0.36)mL·(100 g·min)^(-1)高于对照组(40.57±0.33)mL·(100 g·min)^(-1),试验组GCS评分、NIHSS评分、中医证候积分、GOS评分较对照组明显好转(P<0.05)。结论补阳还五汤治疗气虚血瘀型脑出血可明显减轻血肿周缘区域低灌注改变,有利于恢复脑血供,促进神经功能恢复,改善患者预后。Objective To observe the effect of Buyang Huanwu Decoction(补阳还五汤)on cerebral blood flow parameters in patients with cerebral hemorrhage of Qi deficiency and blood stasis type.Methods From January 2020 to June 2022,80 patients with basal ganglia thalamic cerebral hemorrhage of Qi deficiency and blood stasis type were randomly divided into experimental group and control group,with 40 patients in each group.The experimental group was treated with Buyang Huanwu Decoction for 14 days on the second day of admission based on conventional drug treatment.Within 24 hours and 21 days of admission,magnetic resonance perfusion weighted imaging(PWI)was used to observe the changes of cerebral blood flow parameters in the two groups at the outer edge of the largest layer of hematoma 1 cm below the cortex.Results After 21 days of treatment,the local cerebral blood flow at the outer edge of the largest hematoma layer 1 cm below the cortex in the experimental group[(57.05±0.36)mL·(100 g·min)^(-1)]was higher than that in the control group[(40.57±0.33)mL·(100 g·min)^(-1)].The Glasgow Coma Scale(GCS)score,National Institute of Health stroke scale(NIHSS)score,TCM syndrome score and Glasgow Outcome Scale(GOS)score in the experimental group were significantly improved compared to those in the control group(P<0.05).Conclusion Buyang Huanwu Decoction can significantly alleviate the changes of hypoperfusion in the peripheral region of hematoma in the treatment of cerebral hemorrhage of Qi deficiency and blood stasis type,which is beneficial to restore the cerebral blood supply,promote the recovery of neurological function and improve the prognosis of patients.
