视频内镜下低温等离子开窗术治疗上颌骨囊肿的效果及对创伤反应的影响

摘　　要：目的探究视频内镜下低温等离子开窗术治疗上颌骨囊肿的效果及对创伤反应的影响。方法选择2019年1月至2022年5月我院收治的78例上颌骨囊肿患者作为研究对象,以随机数字表法将其分为对照组和观察组,每组39例。对照组接受传统手术方案,观察组实施视频内镜下低温等离子开窗术。比较两组的手术疗效相关指标、创伤反应指标、并发症发生情况及复发率。结果观察组的术中出血量少于对照组,手术时间、创面愈合时间及住院时间短于对照组,术后24 h视觉模拟评分法(VAS)评分低于对照组,差异具有统计学意义(P<0.05)。术后24 h,观察组的促肾上腺皮质激素(ACTH)、前列腺素E_(2)(PGE_(2))、促肾上腺皮质激素释放激素(CRH)及C-反应蛋白(CRP)水平均低于对照组,差异具有统计学意义(P<0.05)。观察组在院期间的并发症总发生率及复发率显著低于对照组,差异具有统计学意义(P<0.05)。结论视频内镜下低温等离子开窗术治疗上颌骨囊肿效果显著,不仅可缓解患者创伤反应,还能降低并发症发生率及复发率,值得推广。Objective To explore the effect of low temperature plasma fenestration under video endoscopy in the treatment of maxillary cysts and its influence on wound response.Methods A total of 78 patients with maxillary cysts admitted in our hospital from January 2019 to May 2022 were selected as the research objects and divided into control group and observation group by random number table method,with 39 cases in each group.The control group received traditional surgical procedures,and the observation group underwent low temperature plasma fenestration under video endoscopy.The surgical efficacy related indicators,trauma response indicators,complications and recurrence rate were compared between the two groups.Results The intraoperative blood loss in the observation group was less than that in the control group,the operation time,wound healing time and hospital stay were shorter than those in the control group,the Visual Analogue Scale(VAS)score at 24 h after operation was lower than that in the control group,and the differences were statistically significant(P<0.05).At 24 h after operation,the levels of adrenocorticotrophic hormone(ACTH),prostaglandin E_(2)(PGE_(2)),corticotrophin releasing hormone(CRH)and C-reactive protein(CRP)in the observation group were lower than those in the control group,and the differences were statistically significant(P<0.05).The total incidence of complications during hospitalization and recurrence rate in the observation group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).Conclusion Low temperature plasma fenestration under video endoscopy is effective in the treatment of maxillary cysts.It can not only alleviate the wound response of patients,but also reduce the incidence of complications and recurrence rate.It is worth promoting.
