温胆汤合半夏泻心汤治疗轻中度血管性痴呆临床观察

摘　　要：目的:观察温胆汤合半夏泻心汤治疗轻中度血管性痴呆患者的临床疗效。方法:选取轻中度血管性痴呆患者70例,随机分为两组各35例。两组患者均予基础治疗,对照组予盐酸多奈哌齐片治疗,治疗组予温胆汤合半夏泻心汤治疗,1个月为1个疗程,治疗3个疗程。比较两组患者的临床疗效以及治疗前后简易智能状态检查量表(MMSE)、日常生活能力量表(ADL)评分、中医证候积分及血清MMP水平。结果:治疗组临床总有效率88.57%,明显高于对照组的71.43%(P﹤0.05);治疗后两组MMSE评分均升高(P﹤0.05),且治疗组高于对照组(P﹤0.05);两组ADL评分、中医证候积分及血清MMP-2、MMP-9水平均降低(P﹤0.05),且治疗组降低程度较对照组更明显(P﹤0.05)。结论:温胆汤合半夏泻心汤治疗轻中度血管性痴呆患者疗效明显,可有效改善患者的认知功能和日常生活能力。Objective:To observe the clinical efficacy of Wendan decoction and Banxia Xiexin decoction on mild to moderate vascular dementia.Methods:70 cases of patients with mild to moderate vascular dementia were selected and randomly divided into two groups,35 cases in each group.At first,both groups were given basic treatment.Then,the control group was treated with donepezil hydrochloride tablets,while the treatment group was treated with Wendan decoction and Banxia Xiexin decoction.One month as one course of treatment,and for three courses of treatment.Clinical efficacy as well as mini-mental state examination(MMSE),activities of daily living(ADL),TCM syndrome score and serum MMP level were compared between two groups before and after treatment.Results:The total clinical effective rate of treatment group was 88.57%,which was significantly higher than 71.43%of control group(P﹤0.05).MMSE scores were increased in both groups after treatment(P﹤0.05),and the treatment group was higher than control group(P﹤0.05).ADL score,TCM syndrome score and levels of serum MMP-2 and MMP-9 were decreased in both groups after treatment(P﹤0.05),and the decrease degree of treatment group was more obvious than that of control group(P﹤0.05).Conclusion:Wendan decoction and Banxia Xiexin decoction have significant curative effect on patients with mild to moderate vascular dementia,which can effectively improve their cognitive function and daily living ability.
