补肾活血解毒方联合脐带间充质干细胞移植对急性肝衰竭大鼠的治疗作用

摘　　要：目的:观察补肾活血解毒方联合大鼠脐带间充质干细胞(rUCMSCs)移植对急性肝衰竭(ALF)大鼠的治疗作用。方法:采用两步消化法提取rUCMSCs。使用D-氨基半乳糖/脂多糖(D-gal/LPS)构建ALF大鼠模型,24 h后将存活的40只模型大鼠随机分成中药联合移植组、移植对照组、中药对照组及空白对照组。中药联合移植组和移植对照组予以2×10^(6)个rUCMSCs尾静脉注射,余下两组予以等体积无菌PBS溶液尾静脉注射;中药联合移植组和中药对照组予以补肾活血解毒方灌胃(每次10 mL/kg),其余两组予以等量生理盐水灌胃,每日观察各组大鼠一般情况。治疗后第7天,计算各组大鼠7天生存率,并检测其血清中丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)和总胆红素(TBIL)水平,然后通过苏木-伊红(HE)染色观察其肝、肺组织病理学改变。结果:造模后,模型大鼠生存状态逐渐下降,肝功能明显异常。经治疗后,中药联合移植组、移植对照组、中药对照组、空白对照组生存率分别为70%、50%、40%、20%,中药联合移植组生存率显著高于空白对照组(P﹤0.05);中药联合移植组大鼠血清TBIL、ALT、AST水平显著下降,与移植对照组、中药对照组、空白对照组相比,差异有统计学意义(P﹤0.05);病理检测显示中药联合移植组大鼠肝脏组织明显修复,肝小叶结构完整,炎性细胞浸润明显减少,且未见肺栓塞、肺纤维化等不良反应。结论:补肾活血解毒方联合rUCMCSs移植治疗可明显缓解ALF大鼠肝脏炎症反应,改善肝功能,促进肝再生,提高ALF大鼠的存活率,且治疗效果优于二者单独应用,补肾活血解毒方与rUCMCSs联合应用对ALF的治疗具有协同作用。Objective:To observe the therapeutic efficacy of Bushen Huoxue Jiedu formula and rat's umbilical cord mesenchymal stem cells(rUCMSCs)transplantation on rats with acute liver failure(ALF).Methods:rUCMSCs were extracted by two-step digestion method.ALF rats models were established by D-galactosamine/lipopolysaccharide(Dgal/LPS).After 24 hours,40 surviving model rats were randomly divided into TCM and transplantation group,transplantation control group,TCM control group and blank control group.TCM and transplantation group and transplantation control group were injected with 2×10^(6) rUCMSCs through tail vein,while remaining two groups were injected with equal volume of aseptic PBS solution through tail vein.In addtion,TCM and transplantation group and TCM control group were given Bushen Huoxue Jiedu decoction(10mL/kg)by intragastric administration,while remaining two groups were given equal volume of normal saline by intragastric administration.Meanwhile,the general conditions of rats in each group were observed daily.On the 7th day after treatment,the 7-day survival rate of rats was calculated in each group,and the levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST)and total bilirubin(TBIL)in serum were detected.Then,the histopathological changes of liver and lung were observed by hematoxylin-eosin(HE)staining.Results:After modeling,the survival state of model rats was declined gradually,and the liver function was significantly abnormal.After treatment,the survival rates in TCM and transplantation group,transplantation control group,TCM control group and blank control group were 70%,50%,40%and 20%respectively.The survival rate of TCM and transplantation group was significantly higher than that of blank control group(P﹤0.05).The levels of TBIL,ALT and AST in serum of rats in TCM and transplantation group were decreased significantly,and the differences were statistically significant compared with remaining three groups(P﹤0.05).Pathological examination showed that the liver tissue of rats in
