摘 要:目的分析狼疮肾炎(LN)患者T淋巴细胞、转化生长因子-β(TGF-β)水平与疾病活动度的相关性。方法回顾性分析2018年9月至2023年9月在咸阳市中心医院进行治疗的160例系统性红斑狼疮(SLE)患者的临床资料,其中LN患者100例,无LN患者60例。另选同期于咸阳市中心医院体检中心体检健康的50例志愿者设为对照组,比较所有受试者T淋巴细胞[白细胞分化抗原3(CD3^(+))、白细胞分化抗原4(CD4^(+))、辅助性T细胞22(Th22)]及TGF-β水平,比较不同疾病活动度、不同预后LN患者的T淋巴细胞、TGF-β水平,采用Spearman相关系数分析T淋巴细胞、TGF-β水平与系统性红斑狼疮疾病活动度(SLEDAI)评分的相关性。结果LN组血清CD3^(+)、CD4^(+)水平低于无LN组,Th22、TGF-β水平高于无LN组,无LN组CD3^(+)、CD4^(+)水平低于对照组,Th22、TGF-β水平高于对照组(F=217.170、311.154、84.268、56.952,均P<0.05);不同疾病活动度LN患者血清CD3^(+)、CD4^(+)水平比较,稳定期>轻度活动度>中度活动度>重度活动度,Th22、TGF-β水平比较,稳定期<轻度活动度<中度活动度<重度活动度(F=24.672、41.285、28.861、11.130,均P<0.05);预后良好组血清CD3^(+)、CD4^(+)水平高于预后不良组,Th22、TGF-β水平低于预后不良组(t=6.543、9.974、14.230、2.703,均P<0.05);相关性分析显示,CD3^(+)、CD4^(+)水平与SLEDAI评分呈负相关,Th22、TGF-β水平与SLEDAI评分呈正相关(r=-0.458、-0.597、0.651、0.531,均P<0.05)。结论T淋巴细胞、TGF-β异常表达与LN患者疾病活动度密切相关,临床诊治时可结合以上指标共同判断患者病情严重程度及预后。Objective To analyze the correlation between T lymphocyte and transforming growth factor beta(TGF-β)levels and disease activity in patients with lupus nephritis(LN).Methods A retrospective analysis was conducted on the clinical data of 160 patients with systemic lupus erythematosus(SLE)treated at the Xianyang Central Hospital from September 2018 to September 2023,including 100 LN patients and 60 non LN patients.Another 50 healthy volunteers who underwent physical examination at the Xianyang Central Hospital Physical Examination Center during the same period were selected as the control group.The levels of T lymphocytes[leukocyte differentiation antigen 3(CD3^(+)),leukocyte differentiation antigen 4(CD4^(+)),helper T cell 22(Th22)]and TGF-βwere compared among all subjects.The levels of T lymphocytes and TGF-βin LN patients with different disease activity and prognosis were compared.Spearman correlation coefficient was used to analyze the correlation between T lymphocytes,TGF-βlevels and Systemic Lupus Erythematosus Disease Activity Index(SLEDAI)score.Results The serum levels of CD3^(+)and CD4^(+)in the LN group were lower than those in the non LN group,while the levels of Th22 and TGF-βwere higher than those in the non LN group.The levels of CD3^(+)and CD4^(+)in the non LN group were lower than those in the control group,while the levels of Th22 and TGF-βwere higher than those in the control group(F=217.170,311.154,84.268,56.952,all P<0.05);Comparison of serum CD3^(+)and CD4^(+)levels in LN patients with different disease activities:stable phase>mild activity>moderate activity>severe activity.Comparison of Th22 and TGF-βlevels:stable phase<mild activity<moderate activity<severe activity(F=24.672,41.285,28.861,11.130,all P<0.05);The serum levels of CD3^(+)and CD4^(+)in the group with good prognosis were higher than those in the group with poor prognosis,while the levels of Th22 and TGFβwere lower than those in the group with poor prognosis(t=6.543,9.974,14.230,2.703,all P<0.05);Correlation analysis showed t
