摘 要:目的探讨气道护理对重症患者舒适度及呼吸机相关性肺炎的影响。方法回顾分析2022年6月至2023年12月我科室收治重症行呼吸机治疗者112例,依据患者就诊时间依次将其分为对照组及观察组各56例。对照组予以常规护理,观察组采用针对性气道护理。采用Kolcaba的舒适状况量表(GCQ)评估干预前后两组患者的舒适度,比较干预前后两组患者的肺功能变化情况,同时比较两组患者的呼吸机相关性肺炎(VAP)发生情况。结果经护理干预5d后,两组GCQ评分均有所升高,观察组更高(均P<0.05)。呼吸机撤机后3d,两组患者用力肺活量(FVC)、每分钟最大通气量(VEmax)及最大通气量(MVV)指标均较护理前升高,但观察组更高(均P<0.05)。观察组VAP发生率低于对照组(P<0.05)。结论气道护理对重症监护室行呼吸机辅助通气患者具有确切效果,能够提升患者舒适度,帮助提高肺功能,还有助于降低VAP发生率。Objective To explore the impact of airway care on comfort and ventilator-associated pneumonia in critically ill patients.Methods A retrospective analysis was conducted on 112 critically ill patients treated with mechanical ventilation in our department from June 2022 to December 2023.They were divided into a control group and an observation group,with 56 patients in each group,based on their visit time.The control group received routine care,while the observation group received targeted airway care.Use Kolcaba's Comfort Scale(GCQ)to evaluate the comfort level of two groups of patients before and after intervention,compare the changes in lung function before and after intervention,and compare the incidence of ventilator-associated pneumonia(VAP)between the two groups of patients.Results After 5 days of nursing intervention,although the GCQ scores of both groups were increased,the increase was more significant in the observation group(all P<0.05).3 days after ventilator withdrawal,forced vital capacity(FVC),maximum ventilation per minute(VEmax)and maximal voluntary ventilation(MVV)indexes in 2 groups were higher than those before care,but the observation group was more significant(all P<0.05).The incidence of VAP in observation group was lower than that in control group(P<0.05).Conclusions Airway care has a definite effect on patients undergoing ventilator-assisted ventilation in the intensive care unit,which can improve patient comfort,help improve lung function,and also help reduce the incidence of VAP.
