补肾活血汤联合五点支撑功能锻炼治疗椎体成形术后残余腰背痛临床研究

摘　　要：目的:观察补肾活血汤联合五点支撑功能锻炼治疗椎体成形术后残余腰背痛的临床疗效。方法:选取70例骨质疏松性椎体压缩性骨折且行椎体成形术的患者,按随机数字表法分为对照组和观察组各35例。对照组给予常规西药治疗,观察组在对照组基础上给予补肾活血汤加五点支撑功能锻炼治疗,2组均治疗1个月。评价2组临床疗效,比较2组治疗前后疼痛程度、腰背功能障碍程度及腰椎功能,观察不良反应发生情况。结果:观察组总有效率为91.43%,高于对照组71.43%(P<0.05)。治疗后2周、治疗后1个月,2组视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)评分较治疗前降低(P<0.05);且观察组VAS评分、ODI评分低于同时间点对照组(P<0.05)。治疗后2周、治疗后1个月,2组日本骨科协会评估治疗(JOA)评分较治疗前升高(P<0.05);且观察组JOA评分高于同时间点对照组(P<0.05)。观察组不良反应发生率为2.86%,低于对照组17.14%(P<0.05)。结论:补肾活血汤联合五点支撑功能锻炼治疗椎体成形术后患者疗效较好,可有效改善患者残余腰背痛,提高腰椎功能。Objective:To observe the clinical effect of Bushen Huoxue Decoction combined with five-point support function exercise in the treatment of residual low back pain after vertebroplasty.Methods:A total of 70 cases of patients with osteoporotic vertebral compression fractures who underwent vertebroplasty were selected and randomly divided into the control group and the observation group,with 35 cases in each group,using a random number table method.The control group was treated with conventional western medicine,and the observation group was treated with Bushen Huoxue Decoction combined with five-point support function exercise on the basis of the treatment of the control group.Both groups were treated for one month.Evaluated the clinical effects of the two groups,compared the pain degree,lumbar dysfunction,and lumbar function before and after treatment,and observed the occurrence of adverse reactions.Results:The total effective rate was 91.43%in the observation group,which was higher than the control group's 71.43%(P<0.05).After two weeks and one month of treatment,the visual analogue scale(VAS)scores and Oswestry disability index(ODI)in both groups were decreased when compared with those before treatment(P<0.05);the VAS and ODI scores in the observation group were lower than those in the control group at the same time point(P<0.05).After two weeks and one month of treatment,the Japanese Orthopedic Association(JOA)scores in both groups were raised when compared with those before treatment(P<0.05);the JOA score in the observation group was higher than that in the control group at the same time point(P<0.05).The incidence of adverse reactions was 2.86%in the observation group,lower than that of 17.14%in the control group(P<0.05).Conclusion:The combination of Bushen Huoxue Decoction and five-point support functional exercise has a good therapeutic effect on patients after vertebroplasty,which can effectively improve residual low back pain and improve lumbar function.
