三七粉口服联合自体血回输治疗髋部骨折人工髋关节置换术后患者临床研究

摘　　要：目的:观察三七粉口服联合自体血回输对髋部骨折人工髋关节置换术后患者凝血功能、血液流变学、下肢深静脉血栓(DVT)的影响。方法:选取92例髋部骨折患者,按照随机数字表法分为对照组和观察组各46例。2组均行人工髋关节置换术,对照组术中给予自体血回输治疗,观察组在对照组基础上给予三七粉口服治疗。比较2组治疗前后凝血功能指标[纤维蛋白原(FIB)、凝血酶原时间(TT)、部分活化酶还原时间(APTT)、D-二聚体(D-D)]和血液流变学指标(全血高切黏度、血浆黏度、全血低切黏度、红细胞沉降率(ESR))及治疗期间并发症发生率。结果:治疗期间,对照组剔除3例,观察组剔除4例,最终对照组完成研究43例,观察组完成研究42例。治疗后,2组FIB、D-D、ESR及全血高切黏度、血浆黏度、全血低切黏度水平均较治疗前降低(P<0.05),TT、APTT均延长(P<0.05);观察组FIB、D-D、ESR及全血高切黏度、血浆黏度、全血低切黏度水平低于对照组(P<0.05),TT、APTT长于对照组(P<0.05)。观察组下肢DVT发生率低于对照组(P<0.05)。结论:三七粉口服联合自体血液回输可有效改善髋部骨折患者人工髋关节置换术后的凝血功能及血液流变学指标,降低术后下肢DVT的发生率。Objective:To observe the effect of oral administration of Sanqi Powder combined with autologous blood transfusion on coagulation function,hemorheology and deep vein thrombosis(DVT)in the lower extremity of patients with hip fracture after total hip arthroplasty.Methods:A total of 92 cases of patients with hip fracture were selected and divided into the control group and the observation group according to the random number table method,with 46 cases in each group.Both groups were treated with artificial hip replacement.The control group was treated with autologous blood transfusion during the operation,and the observation group was treated with oral administration of Sanqi Powder on the basis of the control group.The coagulation function indexes[fibrinogen(FIB),thrombin time(TT),activated partial thromboplastin time(APTT),and D-dimer(D-D)]and hemorheology indexes[whole blood high shear viscosity,plasma viscosity,whole blood low shear viscosity,and erythrocyte sedimentation rate(ESR)]before and after treatment and the incidence of complications during treatment were compared between the two groups.Results:During the treatment period,three cases in the control group and four cases in the observation group were eliminated,and finally 43 cases in the control group and 42 cases in the observation group were completed.After treatment,FIB,D-D,ESR,whole blood high shear viscosity,plasma viscosity and whole blood low shear viscosity levels were reduced in the two groups compared with before treatment(P<0.05),and TT,APTT all was prolonged(P<0.05),FIB,D-D,ESR and whole blood high shear viscosity,plasma viscosity,whole blood low shear viscosity levels in observation group were lower than those in control group(P<0.05),TT,APTT was longer than that in control group(P<0.05).The incidence of DVT in the lower extremity in the observation group was lower than that in the control group(P<0.05).Conclusion:The oral administration of Sanqi Powder combined with autologous blood transfusion can effectively improve the coagulation functio
