消胀排气汤联合中药热奄包促进剖宫产术后胃肠功能恢复临床研究

摘　　要：目的:观察消胀排气汤联合中药热奄包对剖宫产术后胃肠功能恢复的增效作用与安全性。方法:将120例剖宫产术后产妇按随机数字表法分为对照组、热奄包组、联合组各40例。对照组术后予常规处理,热奄包组在对照组基础上加用中药热奄包热敷腹部穴位,联合组在热奄包组基础上予消胀排气汤治疗。比较3组产妇术后首次肠鸣音恢复时间、首次排气时间、首次排便时间、术后住院时间、腹胀发生率,术后6 h、24 h、48 h、72 h腹痛视觉模拟评分法(VAS)评分,并观察不良反应发生情况。结果:热奄包组、联合组产妇首次肠鸣音恢复时间、首次排气时间、首次排便时间及术后住院时间均短于对照组(P<0.05)。联合组产妇首次肠鸣音恢复时间、首次排气时间、首次排便时间及术后住院时间均短于热奄包组(P<0.05)。热奄包组、联合组产妇术后腹胀发生率均低于对照组(P<0.05);联合组产妇腹胀发生率低于热奄包组(P<0.05)。术后6 h开始,3组腹痛VAS评分均随着时间推移而下降(P<0.05)。热奄包组、联合组术后6 h、24 h、48 h、72 h腹痛VAS评分均低于对照组(P<0.05)。联合组术后24 h、48 h、72 h腹痛VAS评分均低于热奄包组(P<0.05)。3组不良反应比较,差异无统计学意义(P>0.05)。结论:消胀排气汤联合中药热奄包对剖宫产术后胃肠功能恢复疗效确切,能减轻产妇术后腹部疼痛,促进产妇早日康复。Objective:To observe the synergistic effect and safety of Xiaozhang Paiqi Decoction combined with Chinese medicine hot package on the recovery of gastrointestinal function after cesarean section.Methods:A total of 120 postpartum women who underwent cesarean section were randomly divided into the control group,the hot package group,and the combination group,with 40 cases in each group.The control group received routine treatment after surgery,the hot package group was treated with Chinese medicine hot package on abdominal points in addition to the treatment of the control group,and the combination group was treated with Xiaozhang Paiqi Decoction based on the treatment of the hot package group.Compared the recovery time of the first bowel sounds,the first exhaust time,the first defecation time,postoperative hospitalization time,incidence of abdominal distension,visual analogue scale(VAS)scores of abdominal pain at 6,24,48,and 72 hours after surgery among the three groups of postpartum women,and observed the occurrence of adverse reactions.Results:The recovery time of the first bowel sound,the first exhaust time,the first defecation time,and the postoperative hospitalization time in the hot package group and the combination group were all shorter than those in the control group(P<0.05).The recovery time of the first bowel sound,the first exhaust time,the first defecation time,and the postoperative hospitalization time in the combination group were all shorter than those in the hot package group(P<0.05).The incidence of postoperative abdominal distension in both of the hot package group and the combination group was lower than that in the control group(P<0.05);the incidence of abdominal distension in the combination group was lower than that in the hot package group(P<0.05).Starting from six hours after surgery,the VAS scores for abdominal pain in all the three groups were decreased over time(P<0.05).The VAS scores of abdominal pain in the hot package group and the combination group were lower than those in the contr
