老年糖尿病患者颈动脉超声下颈动脉内膜中层厚度和颈动脉斑块积分与心脏瓣膜钙化的相关性研究

摘　　要：目的探讨老年糖尿病患者颈动脉超声下颈动脉内膜中层厚度(inteima-media thickness,IMT)、颈动脉斑块积分(Crouse积分)与心脏瓣膜钙化的相关性。方法选取2020年3月至2022年3月共青城市人民医院收治的60例老年糖尿病患者作为研究对象,根据是否合并心脏瓣膜钙化分为观察组(合并心脏瓣膜钙化,n=24)与对照组(单纯糖尿病,n=36),另选取同期40名体检健康者作为健康组。统计3组临床资料,比较3组颈动脉超声下IMT、Crouse积分,Spearman相关分析IMT、Crouse积分与心脏瓣膜钙化的相关性,Logistic回归分析老年糖尿病患者心脏瓣膜钙化的影响因素。结果观察组与对照组高血压、高血脂发生率高于健康组,观察组吸烟率高于健康组,差异有统计学意义(P<0.05)。观察组与对照组IMT均厚于健康组,Crouse积分均高于健康组,且观察组IMT厚于对照组,Crouse积分高于对照组,差异有统计学意义(P<0.05)。多因素Logistic回归分析结果显示,IMT、Crouse积分是老年糖尿病患者心脏瓣膜钙化的影响因素(P<0.05)。Spear-man相关分析结果显示,老年糖尿病患者心脏瓣膜钙化与IMT、Crouse积分均呈正相关(P<0.05)。结论老年糖尿病患者颈动脉超声下IMT、Crouse积分与心脏瓣膜钙化呈正相关,且IMT、Crouse积分是心脏瓣膜钙化的影响因素,临床应引起重视。Objective To investigate the correlation of carotid intima-media thickness(IMT)under carotid ultrasound,carotid plaque(Crouse)score and cardiac valve calcification in elderly patients with diabetes.Methods A total of 60 elderly patients with diabetes who were admitted to Gongqingcheng People's Hospital from March 2020 to March 2022 were selected as the research subjects,they were divided into the observation group(cardiac valve calcification,n=24)and the control group(simple diabetes,n=36)according to thepresence or absence of cardiac valve calcifi-cation,meanwhile,40 healthy individuals were selected as the healthy group.Clinical data of the three groups were statistically analyzed,IMT un-der carotid ultrasound and Crouse scores among the three groups were compared,Spearman correlation analysis was conducted to discussthe correla-tion between the IMT,Crouse scores and cardiac valve calcification,Logistic regression analysis was performed to screen the influencing factors of cardiac valve calcification in elderly patients with diabetes.Results The incidence rates of hypertension and hyperlipidemia in the observation group and the control group were higher than those in the healthy group,smoking rate in the observation group was higher than that in the healthy group,the differences were statistically significant(P<0.05).The IMT in the observation group and the control group was thicker than that in the healthy group and the Crouse scores was higher than that in the healthy group,the IMT in the observation group was thicker than that in the control group,and the Crouse scores was higher than that in the control group,the differences were statistically significant(P<0.05).IMT and Crouse scores in the observation group and the control group were higher than those in the healthy group,IMT and Crouse scores in the observation group were higher than those in the control group,the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis results showed that IMT and Crouse scores were fa
