腹腔镜小儿疝囊高位结扎术与开放式小切口手术治疗小儿腹股沟斜疝的临床疗效比较

摘　　要：目的探讨腹腔镜小儿疝囊高位结扎术与开放式小切口手术治疗小儿腹股沟斜疝的临床疗效。方法选取2020年4月至2021年8月萍乡市妇幼保健院收治的86例小儿腹股沟斜疝患儿作为研究对象,按照手术方式不同分为对照组与观察组,每组43例。对照组采用开放式小切口手术治疗,观察组采用腹腔镜小儿疝囊高位结扎术治疗,比较两组临床疗效、手术情况、术后不同阶段的疼痛程度、并发症发生情况和术后复发情况。结果观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);观察组手术时间、下床活动时间、术后排气时间和住院时间均短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后24、36h,观察组视觉模拟评分法(visual analogue scale,VAS)均低于对照组,差异有统计学意义(P<0.05);观察组并发症发生率和复发率均低于对照组,差异有统计学意义(P<0.05)。结论腹腔镜小儿疝囊高位结扎术治疗小儿腹股沟斜疝的效果较开放式小切口手术显著,可缩短手术时间,减轻术后疼痛程度,加快康复进程,还可减少并发症发生率和复发率,值得临床推广应用。Objective To investigate the clinical efficacy of laparoscopic high ligation of hernia sac and open small in cision surgeryin children with indirect inguinal hernia.Methods 86 children with indirect inguinal hernia in Pingxiang Maternal and Child Health Hospital from April 2020 to August 2021 were selected as the research subjects,they were divided into the control group and the observation group according to the operation method,with 43 cases in each froup.The control group was treated with open small incision surgery,and the observation group was treated with high ligation of hernia sac under celioscope,the total effective rate,operation situation,pain degree at different stages after operation,the incidence of complications and postoperative recurrence rate were compared between the two groups.Results The total effective rate in the observation group was higher than that in the control group,the difference was statistically significant(P<0.05);the operation time,ambulation time,exhaust time and hospitalization time in the observation group were shorter than those in the control group,the amount of bleeding was less than that in the control group,the differences were statistically significant(P<0.05);at 24 h and 36 h after operation,the visual pain simulation score(VAS)in the observa-tion group were lower than those in the control group,the differences were statistically significant(P<0.05);the complication rate and recurrence rate in the observation group were lower than those in the control group,the differences were statistically significant(P<0.05).Conclusion Laparo-scopic high ligation of hernia sac in children is more effective than open small incision in the treatment of indirect inguinal hernia in children.It can not only shorten the operation time,reduce the postoperative pain,speed up the rehabilitation process,but also reduce the occurrence of complica-tions and the recurrence rate,which is worthy of clinical promotion and application.
