基于经络辨证的首尾取穴法联合冲击波治疗对肩关节周围炎患者肩关节功能的影响

摘　　要：目的观察基于经络辨证的首尾取穴法联合冲击波治疗对肩关节周围炎(以下简称肩周炎)患者肩关节功能的影响。方法选取2022年1月—2023年1月收治的80例肩周炎患者,按照随机数字表法分为对照组(基础针灸治疗+冲击波治疗)与观察组(在上述对照组治疗基础上,根据肩痛位置与经络循行的关系进行首尾取穴治疗)各40例,比较2组疼痛程度、Fugl-Meyer运动功能评分(FMA)、生活质量、肩关节活动度、肩关节综合评分等功能恢复情况。结果治疗后及1个月后随访发现,观察组疼痛程度明显低于对照组,FMA评分、生活质量评分、肩关节活动度、肩关节综合评分高于对照组,差异有统计学意义(P<0.05)。观察组总有效率97.50%(39/40)高于对照组82.50%(33/40),差异有统计学意义(P<0.05)。结论基于经络辨证的首尾取穴法联合冲击波治疗能够有效改善肩周炎患者运动功能,减轻疼痛程度,提高生活质量,效果显著。Objective To analyze the effect of head and tail acupoint selection based on meridian differentiation combined with shock wave on shoulder ioint funtion for shoulder joint in patients with scapulohumeral periarthritis(SP).Methods Eighty eligible SP patients admitted to our hospital from January 2022 to January 2023 were enrolled,and they were divided into the control group(n=40)and the observation group(n=40)according on the different treatments.All patients were received conventional acupuncture and shock wave treatment,those patients in the observation group additionally recevied head and tail acupoint selection based on meridian differentiation.The aim was to compare baseline data,pain level,FMA score,shoulder range of motion(ROM),overall efficacy of shoulder,the recovery of shoulder function.Results After treatment and 1 month follow-up,the observation group had significantly lower pain level,and higher FMA score,quality of life score,shoulder ROM,and comprehensive score for shoulder function than the control group(P<0.05).The effective rate in the observation group was significantly higher than that in the control group(97.50%[39/40]vs 82.50%[33/40],P<0.05).Conclusion Based on meridian differentiation,head and tail acupoint selection combined with shock wave in SP patients has significant effects,which can effectively improve motor function,reduce pain levels,and improve quality of life,and it is worthy of clinical promotion and application.
