董氏奇穴针法联合火龙罐改善中风偏瘫患者肢体功能、神经功能及实验室指标的研究

摘　　要：目的观察董氏奇穴针法联合火龙罐对中风偏瘫患者肢体功能、神经功能及实验室指标的改善作用。方法将江苏省苏州市中西医结合医院2021年12月至2023年3月收治的86例中风偏瘫患者按照随机数字表法分为对照组及研究组,各43例,对照组采用常规护理干预,研究组在对照组基础上应用董氏奇穴针法联合火龙罐,比较2组患者的肢体功能[采用Fugl-Meyer评定量表(FMA)评定]、神经功能[采用美国国立卫生研究院卒中量表(NIHSS)评定]、日常活动能力[采用改良Barthel指数(BI)评定]、疼痛水平[采用疼痛视觉模拟评分法(VAS)评定]及血清环磷酸鸟苷(cGMP)、环磷酸腺苷(cAMP)水平变化。结果干预前2组患者的肢体功能、神经功能、日常活动能力、疼痛水平及血清cGMP、cAMP水平比较,差异均无统计学意义(P>0.05);干预后研究组患者的BI评分、FMA评分均较对照组高,NIHSS评分、VAS评分均较对照组低,差异均有统计学意义(P<0.05);干预后研究组患者的血清cGMP水平较对照组低,cAMP水平较对照组高,差异均有统计学意义(P<0.05)。结论董氏奇穴针法联合火龙罐能够有效改善中风偏瘫患者的肢体及神经功能,提高患者的日常生活能力,降低患者的疼痛水平,调节患者的血清cAMP、cGMP水平,值得临床推广。Objective To study the improvement effect of Dong's extra-point acupuncture combined with Huolong cupping on physical function,neurological function and laboratory indexes in patients with post-stroke hemiplegia.Methods Eighty-six patients with post-stroke hemiplegia treated in Suzhou Hospital of Integrated Traditional Chinese and Western Medicine from December 2021 to March 2023 were randomly assigned into control group(n=43)and study group(n=43).All patients were managed by conventional nursing intervention,and those in observation group were additionally given Dong's extra-point acupuncture combined with Huolong cupping.The aim was to compare Fugl-Meyer Assessment(FMA)scale score,National Institutes of Health Stroke Scale(NIHSS)score,modified Barthel Index(MBI)score,visual analogue scale(VAS)for pain,Serum cyclic guanosine monophosphate(cGMP),cyclic adenosine monophosphate(cAMP).Results There was no statistically significant difference in physical function,neurological function,daily activity ability,pain level,and serum cGMP and cAMP levels between groups before intervention(P>0.05).After intervention,the study group presented significantly higher BI and FMA scores,and lower NIHSS and VAS scores than the control group(P<0.05).The study group had significantly lower cGMP levels,and higher cAMP levels than the control group after intervention(P<0.05).Conclusion Dong's extra-point acupuncture combined with Huolong cupping in patients with post-stroke hemiplegia can effectively improve physical and neurological functions,enhance daily living ability,reduce pain levels,and regulate serum cAMP and cGMP levels,and it is worthy of clinical promotion.
