髂静脉支架置入治疗左髂静脉压迫综合征的效果

摘　　要：目的探讨髂静脉支架置入治疗左髂静脉压迫综合征(iliac vein compression syndrome,IVCS)的应用效果。方法选取2021年6月—2023年6月天水市第一人民医院收治的40例左IVCS伴深静脉血栓(deep venous thrombosis,DVT)患者为研究对象。根据治疗方式不同分为对照组和观察组,各20例。对照组采取常规溶栓抗凝治疗,观察组采取髂静脉支架置入治疗。比较两组的腿围周围径差、D-二聚体、静脉临床严重程度评分(Venous Clinical Severity Score,VCSS)、静脉通畅率、复发率。结果治疗后,两组膝上15 cm、膝下10 cm腿围周围径差较治疗前下降,且观察组差均低于对照组,差异有统计学意义(P均<0.05)。治疗后,两组D-二聚体水平、VCSS较治疗前均下降,且观察组均低于对照组,差异有统计学意义(P均<0.05)。两组治疗7 d后静脉通畅率比较,差异无统计学意义(P>0.05)。观察组治疗3个月后的复发率为0,治疗6个月后的复发率为0,低于对照组的30.00%(6/20)、45.00%(9/20),差异有统计学意义(P均<0.05)。结论髂静脉支架置入治疗左IVCS效果显著,能减小患、健侧大、小腿周围径差,降低D-二聚体水平,促进静脉病变的恢复,提高血流通畅率。Objective To investigate the application effect of iliac vein stent implantation in the treatment of left iliac vein compression syndrome(IVCS).Methods From June 2021 to June 2023,40 patients with left IVCS complicated with deep venous thrombosis(DVT)admitted to First People's Hospital of Tianshui were selected as the research objects.According to different treatment methods,they were divided into control group and observation group,with 20 cases in each group.The control group was treated with conventional thrombolysis and anticoagulation,and the observation group was treated with iliac vein stent implantation.The difference of leg circumference,D-dimer,Venous Clinical Severity Score(VCSS),venous patency rate and recurrence rate were compared between the two groups.Results After treatment,the difference of leg circumference around 15 cm above the knee and 10 cm below the knee in the two groups was lower than that before treatment,and the observation group was lower than the control group,the difference was statistically significant(both P<0.05).After treatment,the levels of D-dimer and VCSS in the two groups were lower than those before treatment,and the observation group was lower than the control group,the differences were statistically significant(both P<0.05).There was no significant difference in venous patency rate between the two groups after 7 days of treatment(P>0.05).The recurrence rate of the observation group after 3 months of treatment was 0,and the recurrence rate after 6 months of treatment was 0,which was lower than 30.00%(6/20)and 45.00%(9/20)of the control group,the difference was statistically significant(both P<0.05).Conclusion The effect of iliac vein stenting in the treatment of left IVCS is significant,which can reduce the difference between the affected side and the healthy side and the calf circumference,reduce the level of D-dimer,promote the recovery of venous lesions,and improve the blood flow patency rate.
