摘 要:目的探讨产前超声诊断胎儿迷走右锁骨下动脉(aberrant right subclavian artery,ARSA)的价值。方法选取2021年6月—2023年6月宜昌市妇幼保健院收治的80例孕妇为研究对象,所有孕妇均实施二维超声、四维超声检查。以临床综合诊断结果为金标准,分析各两种超声检查的疾病检出情况、诊断效能。结果二维超声检出阳性28例,阴性52例;四维超声检出阳性41例,阴性39例。四维超声检查灵敏度为95.24%(40/42)、特异度为97.37%(37/38)、准确度为96.25%(77/80),高于二维超声检查的52.38%(22/42)、84.21%(32/38)、67.50%(52/80),差异有统计学意义(χ^(2)=19.953、3.933、22.279,P均<0.05)。四维超声对食管后型、食管和气管之间型、气管前方型右锁骨下动脉迷走的诊断符合率高于二维超声检查,差异有统计学意义(P<0.05)。结论在胎儿ARSA诊断中,产前四维超声检查可明确具体畸形情况,为下一步医疗操作提供参考。Objective To explore the value of prenatal ultrasound diagnosis in fetal Aberrant right subclavian artery(ARSA).Methods A total of 80 pregnant women admitted to Yichang Maternity&Child Health care Hospital from June 2021 to June 2023 were selected as the research objects.All pregnant women were examined by two-dimensional ultrasound and four-dimensional ultrasound.The clinical comprehensive diagnosis results were used as the gold standard to analyze the disease detection and diagnostic efficacy of the two ultrasound examinations.Results Two-dimensional ultrasound detected 28 positive cases and 52 negative cases.Four-dimensional ultrasound detected 41 positive cases and 39 negative cases.The sensitivity,specificity and accuracy of four-dimensional ultrasound were 95.24%(40/42),97.37%(37/38)and 96.25%(77/80),which were higher than 52.38%(22/42),84.21%(32/38)and 67.50%(52/80)of two-dimensional ultrasound,and the differences were statistically significant(χ^(2)=19.953,3.933,22.279,all P<0.05).The coincidence rate of four-dimensional ultrasound in the diagnosis of right subclavian artery vagal in the posterior esophagus,between esophagus and trachea,and anterior trachea was higher than that of two-dimensional ultrasound,and the difference was statistically significant(P<0.05).Conclusion Prenatal four-dimensional ultrasound examination in the diagnosis of fetal ARSA can clarify the specific malformation situation and provide reference for determining the next medical operation.
