摘 要:目的探讨气压式循环泵预防缺血性脑卒中后下肢深静脉血栓(deep venous thrombosis,DVT)形成的效果。方法选取2022年12月—2023年9月北京中医医院顺义医院收治的68例缺血性脑卒中患者为研究对象,按照治疗方式不同将患者分为对照组及观察组,各34例。对照组接受DVT常规预防治疗;观察组在对照组基础上加入气压式循环泵治疗。对比两组患者的股静脉、腘静脉超声静脉血流参数、下肢皮肤状况、下肢肿胀程度,以及住院期间下肢DVT发生情况。结果治疗前,两组股静脉和腘静脉的峰值流速、平均流速比较,差异无统计学意义(P均>0.05)。治疗后,观察组股静脉的峰值流速、平均流速、腘静脉的峰值流速平均流速均高于对照组,差异有统计学意义(P均<0.05)。观察组下肢肿胀、皮肤颜色及温度变化率均低于对照组,差异有统计学意义(P均<0.05)。治疗前,两组大腿及小腿的周径比较,差异无统计学意义(P均>0.05)。治疗后,观察组大腿及小腿的周径均低于对照组,差异有统计学意义(P均<0.05)。观察组院内下肢DVT发生率为0,低于对照组的11.76%(4/34),差异有统计学意义(P<0.05)。结论气压式循环泵预防缺血性脑卒中后下肢DVT效果显著,可积极改善缺血性脑卒中后下肢静脉血流并降低住院期间下肢DVT发生率。Objective To investigate the effect of pneumatic circulating pump on the prevention of lower extremity deep venous thrombosis(DVT)after ischemic stroke.Methods A total of 68 patients with ischemic stroke who were treated in Beijing Shunyi Hospital of Traditional Chinese Medicine from December 2022 to September 2023 were selected as the research objects.The patients were divided into control group and observation group by different treatment methods,with 34 cases in each group.The control group received routine preventive treatment of DVT,and the observation group were treated with pneumatic circulating pump on the basis of the control group.The ultrasonic venous blood flow parameters of femoral vein and popliteal vein,skin condition of lower limbs,swelling degree of lower limbs,and incidence of DVT of lower limbs during hospitalization were compared between the two groups.Results Before tratment,there were no significant differences in peak flow velocity and average flow velocity of femoral vein and popliteal vein between the two groups at admission(all P>0.05).After tratment,the peak velocity and average velocity of femoral vein,the peak velocity and average velocity of popliteal vein in the observation group were higher than those in the control group,and the differences were statistically significant(all P<0.05).The lower limb swelling,skin color and temperature change rate of the observation group were lower than those of the control group,and the differences were statistically significant(all P<0.05).Before tratment,there were no significant differences in the circumference of thigh and calf between the two groups at admission(both P>0.05).After tratment,the circumference of thigh and calf in the observation group was lower than that in the control group,and the differences were statistically significant(both P<0.05).The incidence of lower extremity DVT in the observation group was 0,which was lower than 11.76%(4/34)in the control group,and the difference was statistically significant(P<0.05).Conclusion Pneu
