红花如意丸对盆腔炎性疾病所致慢性盆腔痛疗效观察

摘　　要：目的观察红花如意丸对盆腔炎性疾病所致慢性盆腔痛患者疼痛程度、炎症反应及子宫动脉血流动力学的影响。方法选择2022年9月-2023年9月郑州大学第一附属医院中医科中西医结合妇科门诊收治的104例盆腔炎性疾病所致慢性盆腔痛患者,按照随机数表法分为治疗组及对照组,各52例。治疗组在常规抗生素治疗基础上加用红花如意丸,对照组将红花如意丸替换为空白模拟剂。比较治疗4周后2组临床疗效,中医证候积分、疼痛程度、生活质量、炎症反应、血液流速及血流动力学变化情况。结果治疗4周后,治疗组总有效率(96.15%)高于对照组(75.00%),差异有统计学意义(P<0.05);2组下腹刺痛、腰骶胀痛、带下异常、经期视觉模拟评分(VAS)、非经期VAS评分,阴道分泌物白细胞酯酶阳性率、全血红细胞沉降率(ESR)、血清白介素-6(IL-6)水平、子宫动脉阻力指数(RI)水平均较治疗前降低,且治疗组优于对照组(P<0.05);2组世界卫生组织生活质量测定简表(WHOQOL-BREF)评分,子宫血流灌注指数(PI)、最大血流速度(PSV)水平较治疗前升高,且治疗组优于对照组(P<0.05)。结论红花如意丸对盆腔炎性疾病所致慢性盆腔痛患者效果较好,可改善患者中医证候积分,缓解疼痛程度,降低炎症反应,调节血液流速及血流动力学,提高生活质量。Objective To study the effects of Honghua Ruyi pill on the pain degree,inflammatory response and uterine arterial hemodynamics in patients with chronic pelvic pain caused by pelvic inflammatory disease.Methods From September 2022 to September 2023,104 patients with chronic pelvic pain caused by pelvic inflammatory disease admitted to the gynecology outpatient at the Department of Integrated Chinese and Western Medicine of the First Affiliated Hospital of Zhengzhou University were enrolled in the treatment group(n=52)and the control group(n=52),using the random number table method.The treatment group was treated with Honghua Ruyi pill on the basis of conventional antibiotics treatment,while in the control group,Honghua Ruyi pill was replaced with a blank simulator.The efficacy of the two groups after 4 weeks of treatment,the changes in their TCM syndrome scores,pain degree,quality of life,inflammatory response,blood flow rate and hemodynamics before and after treatment were compared.Results After 4 weeks of treatment,the total effective rate of the treatment group(96.15%)was significantly higher than that of the control group(75.00%,P<0.05).After 4 weeks of treatment,the scores of lower abdominal tingling,lumbosacral swelling pain,abnormal vaginal discharges,menstrual visual analogue scale(VAS)scores,non-menstrual VAS scores,leukocyte esterase positive rate in vaginal discharges,whole blood erythrocyte sedimentation rate(ESR),serum interleukin-6(IL-6)level and uterine artery resistance index(RI)were all decreased in both groups compared with before treatment,and they were much lower in the treatment group(P<0.05).Compared with before treatment,after 4 weeks of treatment,the scores of the World Health Organization Quality of Life(WHOQOL-BREF),levels of uterine perfusion index(PI)and peak systolic velocity(PSV)were all increased in the two groups,and they were much higher in the treatment group(P<0.05).Conclusion Honghua Ruyi pill can achieve a good curative effect in the treatment of patients with chronic pelvic pa
