摘 要:目的 探究能谱CT及其影像组学对临床T1期肺腺癌淋巴结转移的预测价值。方法回顾性分析经病理证实的154例临床T1期肺腺癌患者。基于双期70 keV单能量图像提取影像组学特征,利用最小绝对收缩和选择算子(LASSO)算法及多因素Logistic回归分析筛选影像组学特征,构建影像组学模型;测量原发肿瘤多项能谱参数,同时结合患者临床特征、CT特征构建临床模型;基于影像组学模型与临床模型构建联合模型并绘制列线图。结果受试者工作特征曲线(ROC)显示联合模型在训练集和测试集中的预测效能最高,曲线下面积(AUC)分别为0.94(95%CI:0.89-0.99)和0.79 (95%CI:0.65-0.92)。决策曲线分析(DCA)结果显示,列线图在一定阈值内的临床效用高于影像组学模型和临床模型。结论基于能谱CT影像组学模型对临床T1期肺腺癌淋巴结转移具有良好的预测价值。Objective To explore the value of radiomics model based on spectral CT to predict lymph node metastasis in clinical stage T1 lung adenocarcinoma.Methods 154 patients with clinical stage of T1lung adenocarcinoma confirmed by pathological diagnosis were analyzed retrospectively.Based on the images of 70 keV single-energy CT values,the least absolute shrinkage and selection operator(LASSO) and multivariate Logistic regression analysis were used to select the radiomics features for constructing radiomics model;several spectral parameters of primary tumor were measured,then clinical model was esta blished by combining the spectral parameters,clinical featu res and CT featu res of patients;the comprehensive model was constructed based on the radiomics model and clinical model and draw the nomogram.Results Receiver operating characteristic(ROC) curve showed that the comprehensive model presented the highest predictive efficiency in the training set and test set,and the a rea under the curve(AUC) values were 0.94(95% CI:0.89-0.99) and 0.79(95% CI:0.65-0.92),respectively.The results of DCA showed that the clinical utility of the nomogram was higher than that in radiomics model and clinical model within a certain threshold ra nge.Conclusion The radiomics model based on spectral CT has a good predictive value for lymph node metastasis in clinical stage T1 lung adenocarcinoma.
