右心房在术后心房颤动复发中的研究进展

摘　　要：心房颤动(房颤)已经日益引起全球的关注,随着年龄的增长,房颤的发病率逐年增加。由于房颤临床治疗策略的不断发展,射频消融术逐渐取代常规药物治疗,已成为房颤的临床一线治疗手段,能大幅度地减少房颤负荷、控制房颤节律及改善心脏功能。然而术后心房颤动复发率仍然是一个重要的临床问题。大量研究证明,房颤持续存在,左心房会发生电生理及解剖结构的重构,这是房颤发生发展和维持以及术后复发的重要病理因素。但近期的研究发现,右心房参与维持心房颤动,并且随着房颤持续时间的增加,右心房与房颤患者的临床结果较差独立相关,对于房颤术后复发的预测作用也存在一定的相关性。本文旨在对右心房与消融术后房颤复发相关性进行综述。Atrial fibrillation(AF)has increasingly attracted global attention,and the incidence of AF increases year by year with age.Due to the continuous development of clinical treatment strategies for AF,radiofrequency ablation has gradually replaced conventional drug therapy and has become a first-line clinical treatment for AF,which can greatly reduce the load of AF and control the rhythm of AF and improve cardiac function.However,the recurrence of postoperative AF is still an important clinical problem.A large number of studies have shown that AF persists,leading to the left atrial electrophysiological and anatomical remodeling occurs,which is an important pathological factor in the development and maintenance of AF and postoperative recurrence.However,recent studies have found that the right atrium is involved in maintaining AF and it is independently associated with the poor clinical outcomes of AF patients with the increase of AF duration;there is also a certain correlation between the right atrium and predictive effect of postoperative recurrence of AF.This article aims to review the correlation between the right atrium and recurrence of AF after ablation.
