摘　　要：目的分析甲状腺激素对A型主动脉夹层患者预后的影响。方法选取2010年1月-2022年12月间上海长海医院收治的390例A型主动脉夹层手术患者,分为死亡组和非死亡组,分析甲状腺激素等对其病死率的影响。结果患者平均年龄(52.1±13.0)岁,男性79.7%;高血压66.9%,肾功能不全39.5%;术中平均体外循环时长(144±39.8)min,术后平均住院时长(24.4±16.9)d;死亡组57例,非死亡组333例,病死率14.6%;两组之间游离三碘甲状腺原氨酸(free thyroxine3,FT3)和游离四碘甲状腺原氨酸(free thyroxine4,FT4)存在明显差异(P<0.05);FT3(OR=0.834,P<0.001)和FT4(OR=0.982,P=0.003)两种激素的水平越高患者的病死率越低。结论FT3和FT4是一种有利于A型主动脉夹层患者预后的重要因素。Objective To investigate the impact of thyroid hormones on the prognosis of patients with type A aortic dissection.Methods 390 patients who underwent surgery for type A aortic dissection at Shanghai Changhai Hospital from January 2010 to December 2022 were selected and categorized into the dead group and the surviving group based on the survival outcome.The influence of thyroid hormones on mortality rate was assessed.Results The average age of the patients was(52.1±13.0)years,with 79.7%being male;66.9% had hypertension,and 39.5%had renal insufficiency.The mean duration of cardiopulmonary bypass during the surgery was(144.0±39.8)minutes,and the average postoperative hospital stay was(24.4±16.9)days.There were 57 deaths in the dead group and 333 in the surviving group,resulting in a mortality rate of 14.6%.Significant differences were observed in the FT3 and FT4 between the two groups(P<0.05).The higher the levels of FT3(OR=0.834,P<0.00l)and FT4(OR=0.982,P=0.003),the lower the fatality rate.Conclusion FT3 and FT4 would be the important prognostic factors in patients with type A aortic dissection.
