血清可溶性CD40L与肝细胞生长因子联合检测在急性肺栓塞患者诊断中的应用价值

摘　　要：目的探讨血清可溶性CD40L(sCD40L)、肝细胞生长因子(HGF)联合检测在急性肺栓塞患者诊断中的应用价值。方法选取2021年1月至2022年12月延安市人民医院收治的96例急性肺栓塞患者为肺栓塞组,根据《肺血栓栓塞症的诊断与治疗指南(草案)》相关标准分为大面积肺栓塞组(50例)及非大面积肺栓塞组(46例)。另选取同期于延安市人民医院就诊的90例非急性肺栓塞患者为对照组;收集两组临床资料及空腹血液,采用酶联免疫吸附法检测两组血清中sCD40L、HGF水平;采用Pearson法分析血清sCD40L与HGF水平的相关性;采用受试者工作特征曲线(ROC)分析血清sCD40L、HGF水平对急性肺栓塞的诊断价值。结果肺栓塞组血清sCD40L、HGF水平明显高于对照组(P<0.05);大面积肺栓塞组血清sCD40L、HGF水平明显高于非大面积肺栓塞组(P<0.05)。肺栓塞组患者血清sCD40L与HGF水平呈正相关(r=0.613,P<0.05)。ROC曲线显示,血清sCD40L、HGF诊断急性肺栓塞的曲线下面积(AUC)分别为0.820、0.848,特异度分别为82.2%、82.2%,敏感度分别为72.9%、72.9%,最佳截断值分别为113.48、492.81 ng/L;sCD40L和HGF联合诊断急性肺栓塞的AUC为0.875,特异度为91.1%,敏感度为74.1%。结论急性肺栓塞患者血清sCD40L、HGF水平升高,且二者呈正相关,sCD40L与HGF联合检测可有效提高急性肺栓塞患者的诊断效能。Objective To explore the value of combined detection of serum soluble CD40L(sCD40L)and hepatocyte growth factor(HGF)in the diagnosis of acute pulmonary embolism(PE).Methods A total of 96 patients with acute PE admitted to Yan'an People's Hospital from January 2021 to December 2022 were selected as the PE group.And they were further divided into a massive PE group(n=50)and non-massive PE group(n=46)according to the relevant criteria of the Diagnosis and Treatment Guidelines for Pulmonary Thromboembolism(Draft).And 90 patients with non-acute PE treated in Yan'an People's Hospital during the same period were selected as the control group.The clinical data and fasting blood of all the subjects were collected.The serum levels of sCD40L and HGF were detected by ELISA.The correlation between serum sCD40L and HGF levels was analyzed by Pearson method.The diagnostic value of serum sCD40L and HGF levels for acute PE was analyzed by ROC curve.Results The serum levels of sCD40L and HGF in the PE group were significantly higher than those in the control group(P<0.05).The serum levels of sCD40L and HGF in the massive PE group were significantly higher than those in the non-massive PE group(P<0.05).There was a positive correlation between serum sCD40L and HGF levels in patients with acute PE(r=0.613,P<0.05).ROC curve showed that the AUC of serum sCD40L and HGF levels in the diagnosis of acute PE was 0.820 and 0.848,the specificity was 82.2%and 82.2%,the sensitivity was 72.9%and 72.9%,and the optimal cut-off value was 113.48 ng/L and 492.81 ng/L,respectively.The AUC of the combined detection of serum sCD40L and HGF levels in the diagnosis of acute PE was 0.875,the specificity was 91.1%,and the sensitivity was 74.1%.Conclusion The serum levels of sCD40L and HGF are elevated in patients with acute PE,and there is a positive correlation between the two.The combined detection of serum sCD40L and HGF levels can effectively improve the diagnostic efficiency of patients with acute PE.
