摘 要:目的探究孕期暴露于不同空气污染物与不良妊娠结局之间的潜在联系,为降低低出生体重儿的出生率提供依据。方法收集2018年1月至2019年12月延安市区两所三级甲等医院的新生儿信息,共纳入低出生体重儿376例为试验组,足月产新生儿1504例为对照组,通过对孕妇的一般信息、本次妊娠情况等多个因素综合分析,结合大气污染物的暴露数据,分析空气污染物暴露与不良妊娠结局之间的关联性。结果两组在孕周、妊娠期并发症、妊娠期高血压、妊娠期合并症方面比较,差异有统计学意义(P<0.05);试验组孕妇在孕早期PM2.5、PM10、SO2、NO2的暴露剂量和整个孕期PM10、SO2的暴露剂量均明显高于对照组(P<0.05);当对混杂因素进行调整后,只有O3在整个孕期为危险因素(OR=1.011,95%CI:1.001~1.021)。结论延安市区大气污染物O3的暴露增加低出生体重儿的发生风险。Objective To explore the potential relationship between exposure to different air pollutants during pregnancy and adverse pregnancy outcomes,provide evidence for reducing the birth rate of low birth weight infants.Methods The data of newborns from two tertiary hospitals in Yan'an City from January 2018 to December 2019 were collected.A total of 376 low birth weight infants were included in the experimental group and 1504 full-term newborns were included in the control group.The association between exposure to air pollutants and adverse pregnancy outcomes was analyzed through comprehensive analysis of multiple factors such as the general information of pregnant women,the pregnancy situation and the exposure data of air pollutants.Results There were significant differences in gestational age,gestational complications,gestational hypertension and gestational comorbidities between the experimental group and the control group(P<0.05).The exposure dose of PM2.5,PM10,SO2 and NO2 in the first trimester and the exposure dose of PM10 and SO2 in the whole pregnancy of the experimental group were significantly higher than those of the control group(P<0.05).After adjusting for confounders,only O3 was a risk factor throughout pregnancy(OR=1.011,95%CI:1.001-1.021).Conclusion The exposure to atmospheric pollutant O3 in Yan'an City increases the risk of low birth weight infants.
