突发性聋伴眩晕患者视频头脉冲实验及温度试验检查结果分析

摘　　要：目的分析单纯突发性聋与突发性聋伴眩晕患者的视频头脉冲试验(video head impulse test,vHIT)检查和温度试验的结果及其特征,为患者的前庭功能评价提供依据。方法收集2017年9月至2018年3月就诊于中国人民解放军第四六三医院耳鼻咽喉科,48例突发性聋伴眩晕患者为实验组,59例单纯突发性聋患者为对照组。突发性聋的诊断符合中华医学会制定的诊断依据,神经学检查无中枢病变体征。对实验组和对照组患者分别进行听力学检查、温度试验及vHIT检查,并对结果进行记录。结果实验组vHIT阳性率为35.4%(17/48),其对应增益值为0.65±0.22,对照组中vHIT阳性率为5.1%(3/59),其对应增益值为0.85±0.19,差异比较有统计学意义(P<0.05)。实验组温度试验阳性率为70.8%(34/48),对照组温度试验阳性率为37.3%(22/59),差异比较有统计学意义(P<0.05)。结论vHIT与温度试验结合可以更加全面地评估突发性聋患者前庭功能损伤的属区情况,且突发性聋伴眩晕患者的前庭功能损伤更严重。Objective To analyze the results and characteristics of the video head impulse test(vHIT)and caloric test in patients with sudden sensorineural hearing loss(SSHL)alone and those with SSHL accompanied by vertigo in order to providing a basis for the clinical evaluation of vestibular function in these patients.Methods A total of 48 patients with SSHL and vertigo who visited the Department of Otolaryngology at the 463rd Hospital of the Chinese People's Liberation Army from September 2017 to March 2018 were included as the experimental group,and 59 patients with SSHL alone served as the control group.The diagnosis of SSHL conformed to the diagnostic criteria established by the Chinese Medical Association,with no neurological examination signs of central lesions.Audiological examinations,caloric tests,and vHIT were performed on both the experimental and control groups,and the results were recorded.Results The positive rate of vHIT in the experimental group was 35.4%(17/48),with a corresponding gain value of 0.65±0.22,while in the control group,the positive rate of vHIT was 5.1%(3/59),with a corresponding gain value of 0.85±0.19.The difference was statistically significant(P<0.05).2.The positive rate of the caloric test in the experimental group was 70.8%(34/48),and the abnormal positive rate of the caloric test in the control group was 37.3%(22/59),with a statistically significant difference(P<0.05).Conclusions The combination of vHIT and temperature test allows for a more comprehensive evaluation of vestibular function impairment in patients with sudden deafness,and the severity of vestibular function impairment is greater in patients with sudden deafness accompanied by vertigo.
