多次输血产生抗-C、抗-JK^(b)联合抗-Wr^(a)致疑难配血1例

摘  要:目的研究1例多次输血患者疑难配血的原因,探讨如何降低多次输血患者产生同种抗体的风险。方法结合临床资料分析,患者血清抗体筛查及鉴定、聚乙二醇抗球蛋白实验及二巯基乙醇法等血清学检测抗体特异性。结果患者血清不规则抗体经鉴定为抗-C、抗-Wr^(a),采用聚乙二醇抗球蛋白加强实验后鉴定出患者血清中还存在抗-Jk^(b)。二巯基乙醇法区分出抗-Wr^(a)为IgM型,效价为16,抗-C效价为8,抗-Jk^(b)效价<1。结论发现1例复合抗-C、抗-Jk^(b)、抗-Wr^(a)引起的疑难配血,通过加强实验提高了Kidd系统抗体的检测效率,及时为患者提供了相匹配的血液。Objective To study the reasons for ambiguous blood matching in a patient who received multiple blood transfusions,and explore how to reduce the risk of producing homologous antibodies in patients who received multiple blood transfusions.Methods The antibody specificity was detected by serum antibody screening and identification,polyethylene glycol(PEG)immunoglobulin assay and dimercaptoethanol method based on clinical data analysis.Results The unexpected antibodies in the patient′s serum were anti-C and anti-Wr^(a),and anti-Jk^(b) was detected after using polyethylene glycol immunoglobulin enhancement experiments.The anti-Wr^(a) was detected as IgM by dimercaptoethanol method,with a titer of 16,anti-C of 8,and anti-Jk^(b)<1.Conclusion One case of ambiguous blood matching caused by anti-C,anti-Jk^(b) combined with anti-Wr^(a) was discovered.By strengthening the experiments,the detection efficiency of Kidd system antibodies was improved,enabling timely provision of matching blood to patients.
