下缘小切口“V”形辅助皮瓣结合打包固定法治疗腋臭的临床疗效观察

摘　　要：目的:探索腋窝下缘小切口“V”形辅助皮瓣剪除顶泌汗腺结合打包固定法治疗腋臭的临床应用及疗效。方法:将2021年8月-2023年8月笔者医院整形美容科手术治疗的80例腋臭患者作为研究对象,采用肿胀麻醉下腋窝下缘小切口“V”形辅助皮瓣剪除顶泌汗腺结合打包固定法治疗,对手术治疗效果随访观察。结果:80例患者均一次性治愈,手术治疗总有效率100%,术后发生并发症者3例,2例于术后第3天出现双侧皮下血肿,另1例拆包时发现双侧切口局部皮肤坏死,均为男性,并发症发生率为3.75%,术后进行3-24月随访,无失访病例,无复发病例,随访患者均无明显瘢痕增生。结论:肿胀麻醉下腋窝下缘小切口“V”形辅助皮瓣剪除顶泌汗腺结合打包固定法治疗腋臭,能避免皮下血肿、皮瓣坏死及术后复发这些常见并发症,治疗效果好,手术方法简单,且切口瘢痕隐蔽,皮肤色泽度基本与正常皮肤一致,达到患者治疗与美观的要求,值得推广。Objective:To explore the clinical application and efficacy of a small incision“V”-shaped auxiliary flap at the lower axillary margin for excising apocrine sweat glands combined with packing fixation in the treatment of bromhidrosis.Methods:From August 2021 to August 2023,80 patients with bromhidrosis who underwent surgical treatment in the Plastic and Aesthetic Department of our hospital were included in this study.The apocrine sweat glands were excised using a small incision“V”-shaped auxiliary flap under local anesthesia at the lower edge of the axilla,combined with the packing fixation method.Results:All 80 patients were cured in a single procedure,resulting in a total effective rate of 100%.Three cases experienced postoperative complications,including two cases of bilateral subcutaneous hematoma on the third day after surgery and one case of local skin necrosis at the bilateral incisions during unpacking,all in male patients.The complication rate was 3.75%.After 3 to 24 months of postoperative follow-up,no patients were lost to follow-up,and there were no cases of recurrence or significant scar hyperplasia observed among the follow-up patients.Conclusion:The treatment of bromhidrosis using a small incision“V”-shaped auxiliary flap at the lower edge of the axilla under local anesthesia effectively avoids common complications such as subcutaneous hematoma,flap necrosis,and postoperative recurrence.This method demonstrates good treatment outcomes,is straightforward to perform,results in concealed scars,and maintains skin color similar to that of normal skin,fulfilling patients’aesthetic and treatment expectations.This technique is worthy of promotion.
