改良法颈内静脉导管固定干预对造血干细胞移植患儿导管意外脱出及堵塞的预防作用

摘　　要：目的探讨改良法颈内静脉导管固定干预在造血干细胞移植(HSCT)患儿护理中的应用。方法选择2019年10月至2022年10月于本院接受颈内双腔静脉置管的76例HSCT患儿作为研究对象,采用随机数字表法将其分为对照组和观察组,各38例。对照组行常规固定法固定,观察组采用改良法颈内静脉导管固定干预。比较两组的干预效果。结果观察组的穿刺点局部感染、接触性皮炎程度均轻于对照组,差异具有统计学意义(P<0.05)。观察组未发生导管意外脱出,且导管堵塞后再通率、导管永久堵塞率低于对照组,差异具有统计学意义(P<0.05)。观察组患儿活动度优于对照组,家属满意度高于对照组,差异具有统计学意义(P<0.05)。结论患儿接受HSCT治疗期间行改良法颈内静脉导管固定干预可获得更理想的固定效果及安全性,有效预防导管意外脱出、堵塞,有助于保证治疗顺利进行,可明显提升家属满意度。Objective To explore the application of modified internal jugular vein catheter fixation in the nursing of children with hematopoietic stem cell transplantation(HSCT).Methods From October 2019 to October 2022,76 children with HSCT who underwent internal jugular double vena cava catheterization in our hospital were selected as the research objects.The children were divided into control group and observation group by random number table method,with 38 cases in each group.The control group was fixed with conventional fixation method,and the observation group was fixed with modified internal jugular vein catheter.The intervention effects of the two groups were compared.Results The degree of local infection at the puncture point and contact dermatitis in the observation group were lighter than those in the control group,and the differences were statistically significant(P<0.05).There was no accidental catheter prolapse in the observation group,and the recanalization rate after catheter blockage and permanent catheter blockage rate were lower than those in the control group,the differences were statistically significant(P<0.05).The activity of children in the observation group was better than that in the control group,and the satisfaction of family members was higher than that in the control group,the differences were statistically significant(P<0.05).Conclusion The modified internal jugular vein catheter fixation intervention during HSCT treatment can achieve better fixation effect and safety,effectively prevent accidental catheter prolapse and blockage,help to ensure the smooth progress of treatment,and significantly improve the satisfaction of family members.
