生肌促愈汤联合中药苦槐熏洗剂对混合痔术后恢复、肛肠动力学及炎性因子的影响

摘　　要：目的探讨生肌促愈汤联合中药苦槐熏洗剂对混合痔术后恢复、肛肠动力学及炎性因子的影响。方法选取2022年1月至2023年6月洛阳市妇幼保健院收治的120例混合痔术后患者作为研究对象,按随机数表法分为联合组和对照组各60例。两组患者术后均给予常规处理,在此基础上,对照组患者给予中药苦槐熏洗剂治疗,联合组患者给予生肌促愈汤联合中药苦槐熏洗剂治疗,连续治疗两周。比较两组患者治疗两周后的疗效,以及治疗前后的创面分泌物量、疼痛、水肿、肉芽生长评分、肛肠动力学指标[直肠最大耐受容量(MTV)、肛管最大收缩压(AMCP)、直肠肛管抑制反射阈值(AIRT)、直肠静息压(RRP)]和炎性因子[超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)]水平,同时比较两组患者的创面愈合时间及治疗两周后的创面愈合率。结果联合组患者的治疗总有效率为95.00%,明显高于对照组的80.00%,差异有统计学意义(P<0.05);治疗两周后,联合组患者的创面分泌物量、疼痛、水肿、肉芽生长评分分别为(0.61±0.14)分、(1.55±0.37)分、(0.53±0.11)分、(0.42±0.10)分,明显低于对照组的(0.78±0.19)分、(1.94±0.52)分、(0.67±0.13)分、(0.55±0.14)分,差异均有统计学意义(P<0.05);治疗两周后,联合组患者的MTV水平(141.25±20.07)mL,明显低于对照组的(164.38±22.51)m L,AMCP、RRP、AIRT水平分别为(13.52±0.66)kPa、(9.13±1.21)mmHg、(32.35±5.03)m L,明显高于对照组的(12.81±0.62)kPa、(7.55±1.10)mmHg、(27.79±4.26)m L,差异均有统计学意义(P<0.05);治疗两周后,联合组患者的血清IL-6、hs-CRP、TNF-α水平分别为(12.25±3.13)ng/L、(8.23±2.03)mg/L、(1.20±0.27)ng/mL,明显低于对照组的(15.68±4.05)ng/L、(11.04±2.41)mg/L、(1.78±0.33)ng/mL,差异均有统计学意义(P<0.05);联合组患者的创面愈合时间为(12.86±2.54)d,明显短于对照组的(18.31±3.16)d,创面愈合率为88.33%,明�Objective To explore the effects of Shengji Cuyu Decoction combined with traditional Chinese medicine Sophora japonica fumigant on postoperative recovery,anorectal dynamics,and inflammatory factors in patients with mixed hemorrhoids.Methods A total of 120 patients with mixed hemorrhoids who underwent surgery at Luoyang Maternal and Child Health Hospital from January 2022 to June 2023 were selected as the research subjects.According to the random number table,they were divided into a combined group and a control group,with 60 patients in each group.On this basis of routine treatment after surgery,patients in the control group were given traditional Chinese medicine Sophora japonica fumigant,and those in the combined group were given Shengji Cuyu Decoction combined with traditional Chinese medicine Sophora japonica fumigant,continuously for 2 weeks.The clinical efficacy after treatment was compared between the two groups,as well as the amount of wound secretion,pain,edema,granulation growth scores,anorectal dynamics(maximum tolerance volume[MTV],anal maximal contraction pressure[AMCP],rectoanal inhibitory reflex threshold[AIRT],rectal resting pressure[RRP]),inflammatory factors(high-sensitivity C-reactive protein[hs-CRP],tumor necrosis factor-α[TNF-α],interleukin-6[IL-6])levels before and after treatment.The wound healing time and the wound healing rate after 2 weeks of treatment were also compared between the two groups.Results The total effective rate of the combined group was 95.00%,which was significantly higher than 80.00%of the control group(P<0.05).After 2 weeks of treatment,the scores for wound secretion volume,pain,edema,and granulation growth in the combined group were(0.61±0.14)points,(1.55±0.37)points,(0.53±0.11)points,and(0.42±0.10)points,respectively,which were significantly lower than(0.78±0.19)points,(1.94±0.52)points,(0.67±0.13)points,and(0.55±0.14)points in the control group(P<0.05).After 2 weeks of treatment,the MTV level in the combined group was(141.25±20.07)mL,significantly lower th
