艾灸联合腕踝针治疗腰椎间盘突出症急性期的临床研究

摘　　要：目的:研究艾灸联合腕踝针治疗腰椎间盘突出症急性期的疗效。方法:将112例腰椎间盘突出症急性期患者随机分为观察组(采用艾灸联合腕踝针治疗,56例)及对照组(腕踝针治疗,56例),比较两组患者的临床疗效、中医证候积分、疼痛程度[视觉模拟量表(VAS)]、腰椎功能[腰椎功能评估量表(JOA)]、功能障碍程度[Oswestry功能障碍指数(ODI)]及腰背肌力参数[屈曲过程表面肌电值(AEMG)、伸直过程AEMG和屈伸松弛比(FRR)]。结果:治疗2周后,观察组患者治疗总有效率较对照组高,差异具有统计学意义(P<0.05);治疗2周后,两组患者中医证候积分均降低,且观察组较对照组低,差异具有统计学意义(P<0.05);治疗1、2周后,两组患者VAS评分均降低,且观察组较对照组低,差异具有统计学意义(P<0.05);治疗2周后,两组患者JOA评分均升高,且观察组较对照组高,差异具有统计学意义(P<0.05);治疗2周后,两组患者ODI评分均降低,且观察组较对照组低,差异具有统计学意义(P<0.05);治疗2周后,两组患者屈曲过程AEMG及FRR均降低,且观察组较对照组低,差异具有统计学意义(P<0.05),伸直过程AEMG均升高,且观察组较对照组高,差异具有统计学意义(P<0.05);治疗2周后,两组患者TNF-α、CRP及IL-6水平均降低,且观察组更低,差异具有统计学意义(P<0.05)。结论:采用艾灸联合腕踝针治疗腰椎间盘突出症急性期患者可减轻其疼痛程度,且疗效较好,同时改善其腰椎功能及腰背肌力,减轻炎症反应,促进其功能恢复。Objective:To study the efficacy of moxibustion combined with wrist-ankle acupuncture in treating acute stage of lumbar disc herniation(LDH).Methods:112 patients with acute stage of LDH were randomly divided into the observation group and the control group,with 56 patients in each group.The observation group was treated with moxibustion combined with wrist-ankle acupuncture,and the control group was only treated with wrist-ankle acupuncture.Clinical efficacy,scores of TCM symptoms,VAS,JOA and ODI,as well as parameters of AEMG during flexion process,AEMG during extension process and FRR were compared between the two groups.Results:After 2 weeks of treatment,the total effective rate of the observation group was higher than that of the control group(P<0.05);the scores of TCM symptoms were decreased in the two groups,and the scores were lower in the observation group than those in the control group(P<0.05).The VAS scores were decreased in the two groups after 1 week and 2 weeks of treatment,and the scores were lower in the observation group than those in the control group(P<0.05).The JOA scores were increased in the two groups after 2 weeks of treatment,and the scores were higher in the observation group than those in the control group(P<0.05).After 2 weeks of treatment,the ODI score was decreased in the two groups,and the score was lower in the observation group than that in the control group(P<0.05).The AEMG during flexion process and FRR were reduced after 2 weeks of treatment,and the indicators were lower in the observation group than those in the control group(P<0.05);the AEMG during extension process was increased,and the indicator was higher in the observation group than that in the control group(P<0.05).The levels of TNF-α,CRP and IL-6 were decreased in the two groups after 2 weeks of treatment,and the levels were lower in the observation group than those in the control group(P<0.05).Conclusion:Moxibustion combined with wrist-ankle acupuncture can relieve the pain degree in treating acute stage of LDH with go
