球囊压力和形态对三叉神经半月微节球囊压迫术效果评定

摘　　要：目的:探讨三叉神经半月节微球囊压迫(PMC)术中球囊压力大小对术后效果的评定,可减少术后复发和再次手术几率。经皮穿刺微球囊压迫(Percutaneous Microcompression,PMC)技术是当前治疗原发性三叉神经痛诸多方法中较常见一种,由Mullan等人于1983年首次报道。方法:本研究于2018年7月至2020年1月采用PMC技术治疗的32例原发性三叉神经痛患者的临床资料进行回顾性分析。其中男性15例,女性17例,年龄85岁以上2例,60-85岁28例,60岁以下2例。结果:根据患麻醉清醒后面部疼痛和感觉麻木等情况判断手术成功率,球囊形态呈理想梨形30例,葫芦形1例,。球形1例。球囊呈梨形且囊内压力大于3KPa(约22mmHg)30例,压力小于2KPa(约15mmHg)2例。其中术后症状立即消失者28例;12小时内症状消失者2例;术后面部疼痛症状无缓解者2例,其中男性1例,女性1例。结论:球囊形态呈梨形,且囊内压力大于3KPa患者术后均有明显效果,形态不理想,且压力小于2KPa患者术后效果均不理想,甚至无效果。Objective:To explore the postoperative evaluation of the effect of balloon pressure during trigeminal ganglion microball compression(PMC),which can reduce the chance of postoperative recurrence and reoperation.Percutaneous Microcompression(PMC)technology is one of the most common methods for the treatment of primary trigeminal neuralgia,first reported by Mullan et al.in 1983.Methods:The clinical data of 32 patients with primary trigeminal neuralgia treated with PMC technology from July 2018 to January 2020 were analysed retrospectively in this study.Among them,there were 15 cases of men,17 cases of women,2 cases over 85 years old,28 cases of 60-85 years old,and 2 cases under the age of 60.Result:According to the condition of anaesthesia,the recovery of the back pain and numbness of the operation are judged.The balloon shape is ideal in 30 cases of pear shape and 1 case of gourd shape.1 case of spherical shape.The balloon is pear-shaped and the pressure in the capsule is greater than 3KPa(about 22mmHg)in 30 cases,and the pressure is less than 2KPa(about 15mmHg)in 2 cases.Among them,28 cases of postoperative symptoms disappeared immediately;2 cases of symptoms disappeared within 12 hours;2 cases of postoperative pain symptoms were not relieved,including 1 male and 1 female.Conclusion:The shape of the balloon is pear-shaped,and the postoperative effect of patients with intracapsular pressure greater than 3KPa is obvious.The morphology is not ideal,and the postoperative effect of patients with pressure less than 2KPa is not ideal,or even no effect.
