摘 要:肺动脉高压是以肺动脉压异常升高为特点的一种致命性疾病,且直接受肺动脉血流动力学影响,肺动脉的血流数值模拟用于临床医疗极具有应用前景。本文基于正常受试者与肺动脉高压患者的电子计算机断层扫描(CT)图像进行三维建模以模拟肺动脉及其复杂分支内血液流动,并进行血流动力学分析。结果发现正常受试者与肺动脉高压患者的肺动脉模型形态具有显著差异,患者左右肺动脉血流分配比失衡,分支处出现异常流动。肺动脉高压患者整体壁面压力较高,肺动脉近端出现大范围的低壁面切应力区,平均壁面切应力低于正常受试者。肺动脉的血流数值模拟可为发病机理及临床诊断提供参考。Pulmonary arterial hypertension is a fatal disease characterized by abnormally elevated pulmonary arterial pressure,which is directly affected by pulmonary artery hemodynamics.The numerical simulation of pulmonary arterial blood flow has great application prospects in clinical medicine.In this paper,three-dimensional modeling was performed to simulate blood flow in the pulmonary artery and its complex branches based on computed tomography(CT)images of normal subjects and patients with pulmonary hypertension,and the hemodynamics was analyzed.There are significant differences in the morphology of the pulmonary artery model between normal subjects and patients with pulmonary arterial hypertension,The patient’s left and right pulmonary artery blood flow distribution ratio is unbalanced,and abnormal flow occurs in the branches.The overall wall pressure of patients is higher,and there is a wide range of low wall shear stress in the proximal pulmonary artery.The mean wall shear stress is lower than that of normal subjects.The numerical simulation of blood flow in pulmonary artery can provide reference for pathogenesis and clinical diagnosis.
