摘 要:目的:分析磁共振弥散张量成像(DTI)在小儿颞叶癫痫中的应用价值。方法:选取2022年1月—2023年12月于青海省妇女儿童医院就诊的68例颞叶癫痫患儿(设为观察组)以及同期于青海省妇女儿童医院进行健康体检的68名未患有癫痫的儿童(设为对照组)为研究对象,均进行DTI检查,比较两组研究对象海马和颞叶白质各向异性分数(FA)、平均弥散系数(MD),分析海马硬化(HS)型与非HS型颞叶癫痫患儿患侧、健侧海马和颞叶白质FA、MD的差异。结果:观察组研究对象海马和颞叶白质患侧MD均高于对照组、FA均低于对照组,差异有统计学意义(P<0.05)。HS型颞叶癫痫患儿患侧海马和颞叶白质MD高于健侧以及非HS型患儿健侧与患侧,差异有统计学意义(P<0.05),而非HS型颞叶癫痫患儿患侧与健侧海马和颞叶白质MD对比,差异无统计学意义(P>0.05);HS型颞叶癫痫患儿患侧海马FA低于健侧以及非HS型患儿健侧与患侧,差异有统计学意义(P<0.05),而HS型和非HS型颞叶癫痫患儿双侧颞叶白质FA对比,差异无统计学意义(P>0.05)。结论:DTI能够有效辅助判断小儿颞叶癫痫病灶位置。Objective Analysing the value of magnetic resonance diffusion tensor imaging(DTI)in paediatric temporal lobe epilepsy.Methods 68 children with temporal lobe epilepsy who received medical treatment in Qinghai Women and Children's Hospital from January 2022 to December 2023(set as the observation group)and 68 children without epilepsy who underwent physical examination in Qinghai Province Women and Children's Hospital during the same period(set as the control group)were selected as the study objects,and DTI examination was performed.The anisotropy fraction(FA)and average dispersion coefficient(MD)of hippocampal and temporal lobe were compared between the two groups,and the differences of FA and MD of hippocampal sclerosis(HS)and non-HS type temporal lobe epilepsy in patients,healthy hippocampus and temporal lobe were analyzed.Results MD on the affected side of hippocampus and temporal white matter in observation group was higher than that in the control group,FA was lower than that in the control group,the difference is statistically significant(P<0.05).The white matter MD of hippocampal and temporal lobe in children with HS type temporal lobe epilepsy was higher than that on healthy side,and healthy side and affected side in children with non-HS type temporal lobe epilepsy,the difference is statistically significant(P<0.05),but there was no statistical significance in the difference between the white matter MD of hippocampus and temporal lobe in children with HS type temporal lobe epilepsy and healthy side(P>0.05).The hippocampal FA on the affected side of children with HS type temporal lobe epilepsy was lower than that on the healthy side,and the healthy side and the affected side of children without HS type temporal lobe epilepsy,the difference is statistically significant(P<0.05),while there was no statistical significance in the comparison of bilateral temporal lobe white matter FA between children with HS type and non-HS type temporal lobe epilepsy(P>0.05).Conclusion DTI can effectively assist in determining
