摘　　要：目的 探究血清胱抑素C(cystatin C, CysC)、同型半胱氨酸(homocysteine, Hcy)、尿素氮(blood urea nitrogen, BUN)及尿微量白蛋白(urinary microalbumin, UmAlb)联合检测在2型糖尿病肾病早期诊断中的临床价值。方法 选取2021年1月—2023年12月济宁市市直机关医院收治的82例2型糖尿病肾病患者及50例单纯2型糖尿病患者作为观察组及对照组。两组均进行CysC、Hcy、BUN、UmAlb检测,比较两组各指标差异,并绘制受试者操作特征(receiver operating characteristic, ROC)曲线分析各指标在2型糖尿病肾病诊断中的价值。结果 观察组CysC、Hcy、BUN、UmAlb水平高于对照组,差异有统计学意义(P均<0.05)。ROC曲线显示,CysC、Hcy、BUN、UmAlb联合检测诊断2型糖尿病肾病的曲线下面积为0.977,且联合检测诊断效能高于单一指标检测。结论 CysC、Hcy、BUN、UmAlb联合检测在2型糖尿病肾病早期诊断价值高,有助于早期识别肾脏损伤。Objective To explore the clinical value of combined detection of serum cystatin C(CysC),homocysteine(Hcy),blood urea nitrogen(BUN)and urinary microalbumin(UmAlb)in the early diagnosis of type 2 diabetic ne-phropathy.Methods From January 2021 to December 2023,82 patients with type 2 diabetic nephropathy and 50 pa-tients with simple type 2 diabetic mellitus were selected as the observation group and the control group.Both groups were tested for CysC,Hcy,BUN,and UmAlb,and the differences in various indicators between the two groups were compared.Receiver operating characteristic(ROC)curves were drawn to analyze the value of each indicator in type 2 diabetic nephropathy diagnosis.Results The levels of CysC,Hcy,BUN and UmAlb in observation group were higher than those in control group,and the differences were statistically significant(all P<0.05).ROC curve showed that the area under the curve of combined detection of CysC,Hcy,BUN and UmAlb was 0.977,and the diagnostic efficiency of combined detection was higher than that of single index detection.Conclusion The combined detection of CysC,Hcy,BUN,and UmAlb has high diagnostic value in the early stage of type 2 diabetic nephropathy,which helps to identify renal injury in the early stage.
